Hyper IgE syndromes (HIES) are primary immunodeficiency disorders of unknown pathogenesis. Patients are typically affected with ‘cold’ abscesses of the skin, recurrent cyst-forming pneumonia, chronic mucocutaneous candidiasis and other less frequent features such as progressive skeletal abnormalities. Defective signaling in the Toll-like receptor (TLR) pathways has been suggested as a responsible pathologic mechanism, however, in previous reports, 10 patients revealed no defect in inflammatory cytokine responses to different TLR ligands. Here, we report the increase in pro-inflammatory cytokines TNF-α and IL-8, following TLR2 and TLR4 stimulation in a larger cohort of 25 additional patients with HIES, and provide a meta-analysis of the TLR data in HIES.

Авторы представляют клинический случай из собственной практики, а также обзор современных литературных данных по данной болезни. Рассмотрены вопросы наследования, патогенеза, клинических проявлений, а также диаг-ностики гипер-IgE-синдрома. Показаны различия в течении болезни при разных типах наследования -аутосомно-доминантном, аутосомно-рецессивном. Ключевые слова: первичный иммунодефицит, гипер-IgE-синдром, наследование, этиология, патогенез, клиническое течение, диагностика, лечение, дети. (Педиатрическая фармакология. 2013; 10 (1): 54-60) Относительно низкая распространенность первич-ных иммунодефицитов (ПИД) в популяции, патогене-тическая разнородность и отсутствие специфических для конкретных нозологических форм клинических маркеров существенно затрудняет их диагностику. В большинстве случаев диагностика ПИД базируется на анализе истории заболевания и жизни больного, данных семейного анамнеза, клинических проявлений иммунной недостаточности и определении иммунного дефекта. Клинические проявления ПИД манифести-руют частыми, тяжело протекающими инфекциями. Поражения респираторного тракта являются наиболее типичными: острые и хронические бронхиты, отиты, синуситы, повторные пневмонии с образованием пнев-матоцеле, плевриты, синуситы протекают тяжело и дли-тельно, антибактериальная терапия имеет временный, непродолжительный эффект [1]. Следует учитывать, что в ряде случаев появление симптомов ПИД рас-тягивается на годы, и это обстоятельство затрудняет своевременную диагностику и лечение. Кроме того, нередко изменения в иммунограмме при ПИД могут в известной степени совпадать с таковыми при отсут-ствии у пациента первичного иммунодефицита. Это особенно характерно для редкого первичного иммуно-дефицита -гипер-IgE-синдрома (синонимы: синдром Бакли, синдром Джоба). При этом заболевании одним из определяющих диагностических признаков заболе-вания является очень высокий уровень сывороточного иммуноглобулина (Ig) E. Однако, известно, что высокий уровень сывороточного IgE встречается при аллергиче-ских, паразитарных и ряде вирусных заболеваниях, что определяет сложности в дифференциальной диагности-ке [2,3]. Кроме того, затрудняет диагностику этого пер-вичного иммунодефицита ряд симптомов, казалось бы далеких от иммунологических нарушений -поражения структур соединительной ткани.

A common human TLR1 polymorphism regulates the innate immune response to lipopeptides

Toll-like receptors (TLR) are critical mediators of the immune response to pathogens and human polymorphisms in this gene family regulate inflammatory pathways and are associated with susceptibility to infection. Lipopeptides are present in a wide variety of microbes and stimulate immune responses through TLR1/2 or TLR2/6 heterodimers. It is not currently known whether polymorphisms in TLR1 regulate the innate immune response. We stimulated human whole blood with triacylated lipopeptide, a ligand for TLR1/2 heterodimers, and found substantial inter-individual variation in the immune response. We sequenced the coding region of TLR1 and found a non-synonymous polymorphism, I602S (base pair T1805G), that regulated signalling. In comparison to TLR1_602S, the 602I variant mediated substantially greater basal and lipopeptide-induced NF-kappaB signalling in transfected HEK293 cells. These signalling differences among TLR1 variants were also found with stimulation by extracts of Mycobacterium tuberculosis. Furthermore, individuals with the 602II genotype produced substantially more IL-6 than those with the 602SS variant in a lipopeptide-stimulated whole-blood cytokine assay. Together, these observations demonstrate that variation in the inflammatory response to bacterial lipopeptides is regulated by a common TLR1 transmembrane domain polymorphism that could potentially impact the innate immune response and clinical susceptibility to a wide spectrum of pathogens.
