Hypercalcaemia, Renal Dysfunction, Anaemia, Bone Disease (CRAB Criteria): A Case of Lymphoma

Abstract: Calcium elevation, Renal dysfunction, Anaemia and Bone disease (CRAB criteria) are usually seen in multiple myeloma (MM). We report a unique case of lymphoma with all the features of CRAB criteria. We describe a 59-year-old patient who presented with confusion, severe back pain, fatigue and constipation and was found to have hypercalcaemia, kidney dysfunction, anaemia and multiple osteolytic lesions. Physical examination and imaging did not reveal any enlarged lymph nodes. Work-up for MM (serum protein electro… Show more

“…The hallmark of MM is the uncontrolled proliferation of plasma cells within the bone marrow, leading to the production of excessive and abnormal immunoglobulins [ 10 13 ]. The disease is often associated with organ dysfunction that manifests as the “CRAB criteria” in clinical practice, including Calcium elevation (C), Renal dysfunction (R), Anemia (A), and Bone disease (B) [ 14 , 15 ].…”

“…Bone lesions, including lytic lesions and osteoporosis, are represented by the "B" in CRAB. If CRAB features are absent, having more than 60% of the cells in the bone marrow as plasma cells can be considered a symptom of MM [8].…”
