Due to increasing antibiotic resistance, the application of antimicrobial photodynamic therapy (aPDT) is gaining increasing popularity in dentistry. The aim of this study was to investigate the antimicrobial effects of aPDT using visible light (VIS) and water-filtered infrared-A (wIRA) in combination with a Hypericum perforatum extract on in situ oral biofilms. The chemical composition of H. perforatum extract was analyzed using ultra-high-performance liquid chromatography coupled with high resolution mass spectrometry (UPLC-HRMS). To obtain initial and mature oral biofilms in situ, intraoral devices with fixed bovine enamel slabs (BES) were carried by six healthy volunteers for two hours and three days, respectively. The ex situ exposure of biofilms to VIS + wIRA (200 mWcm−2) and H. perforatum (32 mg ml−1, non-rinsed or rinsed prior to aPDT after 2-min preincubation) lasted for five minutes. Biofilm treatment with 0.2% chlorhexidine gluconate solution (CHX) served as a positive control, while untreated biofilms served as a negative control. The colony-forming units (CFU) of the aPDT-treated biofilms were quantified, and the surviving microorganisms were identified using MALDI-TOF biochemical tests as well as 16 S rDNA-sequencing. We could show that the H. perforatum extract had significant photoactivation potential at a concentration of 32 mg ml−1. When aPDT was carried out in the presence of H. perforatum, all biofilms (100%) were completely eradicated (p = 0.0001). When H. perforatum was rinsed off prior to aPDT, more than 92% of the initial viable bacterial count and 13% of the mature oral biofilm were killed. Overall, the microbial composition in initial and mature biofilms was substantially altered after aPDT, inducing a shift in the synthesis of the microbial community. In conclusion, H. perforatum-mediated aPDT using VIS + wIRA interferes with oral biofilms, resulting in their elimination or the substantial alteration of microbial diversity and richness. The present results support the evaluation of H. perforatum-mediated aPDT for the adjunctive treatment of biofilm-associated oral diseases.

Bu derlemenin amacı, diş hekimliği alanındaki bitkisel tedavilerin modern bilimin ışığında kullanılabilirliğini irdeleyerek, alternatif tedavi yöntemlerinin klinik kullanım alanlarını araştırmaktır. Gereç ve Yöntemler: PubMed, Cochrane Library, Evidence-Based Dentistry ve Google Scholar'da 2008-2018 yılları arasında yapılan uluslararası yayın taramasında; bitkisel tedaviler, ağız sağlığı, diş hekimliği, sarı kantaron, meyan kökü, sumak, karadut, aloe vera ve bu anahtar kelimelerin farklı kombinasyonları kullanılarak 179 yayına ulaşıldı. Bu yayınlardan, özetlerin okunması sonrasında belirlenen limitler dahilinde antimikrobiyal, antiinflamatuar, analjezik ve antiplak özelliklerinden herhangi birine sahip bitkilerle ilgili olan 50 yayın konu kapsamı içinde incelenmeye uygun görüldü. Bulgular: İncelenen yayınlarda, diş hekimliğinde bitkisel tedavi uygulamalarında analjezik ve antiinflamatuar özellikleri nedeniyle akut ağrıda sarı kantaron, antibakteriyel etkisiyle biyofilm oluşumunu azaltmak için sumak, avülse dişlerde taşıma ortamı olarak karadut, ağız ve diş enfeksiyonlarında karyojenik bakteriler ve perio-patojenlere karşı antimikrobiyal özelliğiyle aloe vera ve meyan kökü kullanıldığı tespit edildi. Sonuç: Tamamlayıcı ve alternatif bitkisel tedavi uygulamalarının, diş hekimliğinde kullanımının yaygınlaşması amacıyla konuya ilişkin bilimsel çalışmaların desteklenmesi gerekmektedir.

St. John's Wort, commonly known as Hypericum perforatum L., is a flowering plant in the Clusiaceae family that traditionally been employed for treating anxiety, depression, wounds, burns, sunburn, irritation, and stomach ailments. This review provides a synopsis of H. perforatum L. phytoconstituents and their biological effects, highlighting its beneficial therapeutic properties for dermatological indications, as well as its antioxidant, antimicrobial, anti‐inflammatory, and anti‐angiogenic activity in various applications including wound healing and skin conditions such as eczema, sun burn and minor burns also spastic paralysis, stiff neck and mood disorders as anti‐depressant and nerve pains such as neuralgia. The data were collected from several databases as Web of Science PubMed, ScienceDirect, Scopus and Google Scholar using the terms: “H. perforatum L.”, “H. perforatum L. /phytochemistry,” and “H. perforatum extracts/wound healing” collected from 1994 to 2023. The findings suggest H. perforatum L. acts through various mechanisms and plays a role in each phase of the wound healing process, including re‐epithelialization, angiogenesis, wound contraction, and connective tissue regeneration. H. perforatum L. enhances collagen deposition, decreases inflammation, inhibits fibroblast migration, and promotes epithelialization by increasing the number of fibroblasts with polygonal shape and the number of collagen fibers within fibroblasts. H. Perforatum L. extracts modulate the immune response and reduce inflammation were found to accelerate the wound healing process via inhibition of inflammatory mediators' production like interleukin‐6, tumor necrosis factor‐α, cyclooxygenase‐2 gene expression, and inducible nitric oxide synthase. Thus, H. perforatum L. represents a potential remedy for a wide range of dermatological problems, owing to its constituents with beneficial therapeutic properties. H. perforatum L. could be utilized in the development of novel wound healing therapies.
