Obstructive sleep apnoea (OSA) is increasing in prevalence due to rising obesity. While OSA is a disorder primarily of the upper airway during sleep, its pathophysiological impact on other body systems is increasingly recognised. There has been interest in the prevalence of OSA in different ophthalmic conditions and possible causation has been postulated. As OSA is common, it can be expected that people with co-existent OSA will be found in any ophthalmic disease population studied. To determine with confidence the significance of finding patients with OSA in a particular cohort requires a well matched control group, ideally matched for age, obesity, gender and co-morbidities. Only if one can say with certainty that the prevalence of OSA is higher in a group with a particular co-existent ophthalmic disease can we begin to speculate about possible mechanisms for the overlap in these conditions. Possible mechanisms for how OSA might affect the eye are discussed in this review. The current literature is reviewed with respect to diabetic retinopathy, glaucoma, floppy eyelid syndrome, non-arteritic ischaemic optic neuropathy, keratoconus and AMD. Associations with OSA have been found, but robust prospective studies using multi-channel sleep studies to diagnose OSA are lacking. Gaps remain in the evidence and in our knowledge. It is hoped that this review will highlight the need for ophthalmologists to consider OSA in their patients. It also makes recommendations for future research, especially to consider whether therapies for OSA can also be effective for ophthalmic disorders.

Purpose: To investigate the Foveal Avascular Zone (FAZ) area in patients with arterial hypertension using optical coherence tomography angiography (OCT-A).Material and methods: The study was permormed at the School Clinic of UNIFACISA and at the Eye Care Center. It was included 21 hypertensive patients (study group) and 19 individuals without hypertension (control group). It was excluded patientes with diabetes or glaucoma. The measurement of the FAZ areas was performed by the DRI OCT-1Triton/Plus. For the quantitative variables, the results were expressed by their means and standard deviations. Qualitative variables were expressed by their absolute and relative frequencies. Student’s t-test was used to verify differences between means and Fisher’s exact test for differences between frequencies. For evaluating the correlation between the superficial and deep plexus areas, the Pearson correlation test was used. P value ≤ 0.05, rejected the null hypothesis. The research was approved by the Ethics Committee of the Medical School from Campina Grande -UNIFACISA and all individuals signed the free and informed consent form.Results: The means of the superficial FAZ of both eyes was significantly higher in the study group (right eye 410.7 ± 160.7 μm2 - 319.1 ± 108.2 μm2 - p = 0.0433); (left eye: 405.4 ± 144.8 μm2 versus 309.4 ± 87.7 μm2- p = 0.0174). The means of the deep FAZ areas of the right and left eyes were similar between the study and control groups (right eye: 559.6 ± 253.28 μm2 versus 605.1 ± 226.8 μm2 - p = 0.5548); (left eye: 673.2 ± 170.8 μm2 versus 585.6 ± 171.2 μm2 - p = 0.1142). The relationships between the superficial and deep plexus areas of both eyes showed a weak correlation between the two plexuses. (r = 0.4734), left eye (r = 0.2287), study group - right eye: (r = 0.4374), left eye (r = 0.3580).Conclusion: The area of superficial FAZ of the right and left eyes was significantly higher in the hypertensive patients. The deep FAZ area of both eyes, however showed similar mean areas between the study and control groups.

АННОТАЦИЯ Цель. Повышение эффективности фармакологической коррекции гипертензивной нейроретинопатии с использованием нового производного диметиламиноэтанола (ДМАЭ) 7-16. Материалы и методы. Моделирование гипертензивной нейроретинопатии проводили путем введения неселективного ингибитора ΝΟ-синтаз N-нитро-L-аргинин-метилового эфира в дозе 12,5 мг/кг в течение 28 суток и повышения внутриглазного давления до 110 мм рт. ст. при оказании механического давления на переднюю камеру глаза. Для исследования глазного дна у экспериментальных животных применена прямая офтальмоскопия (офтальмоскоп Bx a Neitz, Япония). Для увеличения применяли линзу Osher MaxField 78D модель OI-78M. Электроретинографию (ЭРГ) проводили сразу после офтальмоскопии. Регистрацию ЭРГ проводили в ответ на одиночную стимуляцию. Вызванные биопотенциалы усиливались, усреднялись и представлялись графически на экране при помощи Biopac-systems MP-150 с программой AcqKnowledge 4.2 (США). Для оценки степени развития функциональных повреждений сетчатки оценивали соотношение амплитуд b-и а-волн ЭРГкоэффициент b/a. Результаты. Обнаружены выраженные протективные свойства производного ДМАЭ 7-16 в дозе 25 мг/кг/сут, превосходящие его действие в дозе 12,5 мг/кг/сут, заключающиеся в уменьшении развития нейрональных повреждений и сосудистых изменений сетчатки на фоне артериальной гипертензии, которые отмечались в контрольной группе; увеличении коэффициента b/a в группах с коррекцией патологии, что обусловлено восстановлением позитивной волны b на ЭРГ и говорит о сохранении электрофизиологической функции биполярных и мюллеровских клеток. Заключение. Поиск новых путей снижения повреждающего действия ишемии в эксперименте, формирующейся при гипертензивной нейроретинопатии, является актуальной задачей фармакологии, которую можно решить применением нового производного ДМАЭ 7-16.
