ICT and Integrated Care: Some Dilemmas of Standardising Inter-Organisational Communication

Abstract: There is a growing interest in the issues of how to organise healthcare work along individual patient cases rather than along the demarcation lines of healthcare organisations. Health information systems, such as electronic patient records, are seen as important change agents, since they are asserted to help the coordination of care across organisations through fast and accurate exchange of clinical data. The paper explores how a semi-standardised discharge letter is employed to communicate about the patient b… Show more

“…In a series of studies within the healthcare sector published in JCSCW (Aanestad 2003;Winthereik and Vikkelsø 2005;Hanseth and Lundberg 2001;Monteiro 2003, 2006) various authors explicitly draw on an II perspective to analyse how IIs involve a mutual shaping of both technology and work practice. “…Standardisation is embedded in efforts to improve efficiency and quality in health care (Timmermans & Berg, 1997;Winthereik & Vikkelsø, 2005;Klein, 2003). Given the very significant levels of health care expenditure throughout the Western world, the reasons for concern over efficiency are immediately obvious.…”

“…For instance, Bansler and colleagues describe how clinical notes sequentially written by doctors are a coordinative artefact that facilitates collaborative work in treating patients (Bansler et al 2016). Winthereik and Vikkelsø examined the use of discharge letters as bridging devices that support continuity of care and coordination across health providers (Winthereik and Vikkelsø 2005). Østerlund studied the relation between electronic documents, places and collaborative activities in a hospital context.…”
