Identification of Hepatitis B Surface Antigen Variants with Alterations outside the “a” Determinant in Immunized Singapore Infants

Abstract: Vaccine-associated hepatitis B surface antigen (HBsAg) mutations have been mainly identified within the "a" determinant (aa 124-147). Now changes have been detected that are located outside the "a" determinant from immunized Singapore infants born to hepatitis B virus (HBV) carrier mothers. These include Asn116 to Thr116, Val118 to Ala118, Pro120 to Ser120, Ala159 to Val159, Phe183 to Cys183, and Val184 to Ala184. Decreased binding to "a" determinant-specific monoclonal antibody was observed for variants Pro12… Show more

“…Such mutants have also been reported in association with escape from vaccineinduced immunity [Oon et al, 1995;Carman, 1997;Ho et al, 1998;Chong-Jin et al, 1999;He et al, 2001;Lee et al, 2001;Seddigh-Tonekaboni et al, 2001] and with failures of therapy with specific immunoglobulin in newborns from carrier women and in liver-transplant recipients [Harrison et al, 1994;Oon et al, 1996Oon et al, , 2002Brind et al, 1997;Carman, 1997;Ngui et al, 1997;Protzer-Knolle et al, 1998;Santantonio et al, 1999;Roznovsky et al, 2000;Liu et al, 2002]. The amino-acid substitution Gly145Arg has been reported most frequently [Francois et al, 2001], and is known to be stable [Zuckerman and Zuckerman, 1999].…”

“…These mutants may lead to reinfection, because it replicates through an RNA intermediate synthesized by reverse transcriptase of viral genomes (Kreutz, 2002;Ohishi et al, 2004) and quasi-species are generated (Torresi, 2002;Liu et al, 2002;Hsu et al, 2004). This resulted in the production of viral mutants during naturally occurring infections (Chong-Jin et al, 1999). Alterations of the structure of HBsAg can disrupt the binding ability of polyclonal antibodies to it, because they contain several epitopes for T or B cells.…”

“…In Vietnam, three M133L mutations have been found in the first loop of the "a" determinant, no mutation was identified within the second loop, while T126M was found in one sample (Thuy et al, 2005). In Singapore, HBsAg mutation was found in neonates of HBsAg positive mothers; 16 (39%) of 41 neonates showed breakthrough infection despite HBIg and a full course of a plasma-derived vaccine being given (Chong-Jin et al, 1999). However, in Thailand, 22.4% of vaccinated and unvaccinated children were found to have varying mutations in the "a" determinant region of HBV (Poovorawan et al, 2012).…”
