“…2,4,5 Konjenital gerçek lökonişi genellikle otozomal dominant kalıtım paterni gösterir ve izole şekilde tek başına izlenebileceği gibi palmoplantar keratodermi, pilar ve sebase kistler, keratozis pilaris, pili torti, hipotrikozis, onikoreksiz gibi diğer dermatolojik problemler ile de birliktelik gösterebilir. [2][3][4][5][6][7] Palmo plantar keratodermi, sensörinöral işitme kaybı ve herediter lökonişi ile karakterize olan Bart Pumphrey sendromu ya da LEOPARD sendromu gibi hastalıklar ile de ilişkili olabilir. 2,6 Edinilmiş gerçek lökonişilerin ise özellikle punktat tipi tamamen sağlıklı bireylerde de görü-lebilen ve tekrarlayan mikro travmaların etiyolojide önemli rol aldığı bir tablo olup, en sık izlenen lökonişi tipidir.…”

ökonişi, tırnağın bir kısmında ya da tamamında beyaz renk değişik-liğini ifade eder. Hastalığın birçok alt tipi tanımlanmıştır. Gerçek lökonişi olgularında tırnağın beyaz renk değişikliği matriks disfonksiyonu sebebi ile oluşmakta olup, herediter ya da edinsel formları izlenebilir. Edinsel formu genellikle erken çocukluk döneminde ortaya çıkar. 1Lökonişi birçok farklı hastalık ile ilişkili olabileceği gibi izole olarak da izlenebilir. Literatürde bildirilen idiyopatik edinilmiş total gerçek lökonişi olgu sayısı son derece sınırlıdır. Bu çalışmada tüm tırnakların etkilendiği, tırnakların bir kısmını veya tamamını kaplayan beyazlama şikâyeti nedeni ile başvuran ve başka herhangi bir hastalık hikâye ya da bulgusu olmayan bir olgu sunulmuştur.Turkiye Klinikleri J Dermatol 2014;24 (3) 103 İdiyopatik Edinilmiş Parsiyel-Total Lökonişi Ö ÖZ ZE ET T Lökonişi, tırnak plağının beyaz renk değişikliğidir.Gerçek ve yalancı tip lökonişi olarak ayrılır. Gerçek lökonişi hastalarındaki tırnağın beyaz renk değişikliği matriks disfonksiyonu sebebi ile oluşur. Yalancı lökonişi terimi ise tırnak yatağından kaynaklanan beyaz renk değişikliğini ifade etmek için kullanılır. Gerçek lökonişiler edinilmiş ya da herediter olabilir. Edinilmiş tip lökonişi sık-lıkla travma, kemoterapötik ajanlar gibi ilaçlar, sistemik ve lokal enfeksiyonlar, inflamatuar hastalıklar ve maligniteler ile ilişkilidir. Bu çalışmada kalıcı, edinilmiş, parsiyel ve total lökonişisi bulunan 23 yaşındaki bir olgu sunuyoruz. Hastalık ile ilişkisi olabilecek başka bulgu ya da etiyolojik faktör saptanmamıştır. Günümüze kadar literatürde son derece sınırlı sayıda idiyopatik, gerçek, edinilmiş, total lökonişi olgusu bildirilmiştir.A An na ah ht ta ar r K Ke el li im me el le er r: : Tırnak hastalıkları; lökonişi totalis A AB BS ST TR RA AC CT T Leukonychia is the white discoloration of the nail plate. Leukonychia is subdivided as true and pseudo leukonychia. In patients with true leukonychia white discoloration of the nail occurs due to the matrix dysfunction. The term of pseudoleukonychia is used when the white discoloration is caused by the changes of nail bed. True leukonychia may be acquired or inherited. Acquired leukonychia is frequently associated with trauma, drugs such as chemotherapeutic agents, systemic or local infections, inflammatory diseases and malignancies. We present a case of a 23-year-old patient with persistent, true, acquired leukonychia totalis and partialis. We did not detect any other finding or etiological factors that may be associated with disease.
