Idiopathic Multicentric Castleman Disease Occurring Shortly after mRNA SARS-CoV-2 Vaccine

Abstract: Idiopathic Multicentric Castleman Disease (iMCD) is a potentially life-threatening systemic disease whose complex symptomatology is due to cytokine dysregulation. We, herein, present a case of severe iMCD occurring in a previously healthy young man shortly after mRNA SARS-CoV-2 vaccination, responding to interleukin-6 blockade with siltuximab. Six months after the completion of siltuximab, the patient remained without any signs of iMCD or inflammation, indicating a temporal trigger of the disease. This case no… Show more

“…These findings directly indicated that B cell germinal center responses are positively correlated with C19-VAL incidence. Four case reports showed that pathogen reactivation (TB (n = 1) [ 66 ] and EBV(n = 1) [ 89 ]), iMDC (n = 1) [ 90 ] and autoimmune disease (ITP) (n = 1) [ 41 ] are correlated with C19-VAL development. Those findings indicated that reactive change is also related to C19-VAL development.…”

“… 8 Additionally, Andreas Kessler et al and Kazuhiro et al reported instances of iMCD-NOS and iMCD-TAFRO, respectively, occurring in previously healthy young adults shortly after mRNA SARS-CoV-2 vaccination. 9 , 10 David and their team presented findings from their follow-up cohort, which included 10 (14.5%) CD patients who had tested positive for nucleic acid, with the majority of patients exhibiting mild symptoms and no evidence of disease progression. 15 In our study, similar mild symptoms during the acute infection of COVID-19 were observed.…”

“… 5 7 Currently, there have been several case reports that presented new diagnosis of CD after the infection of SARS-CoV-2 in sporadic cases, highlighting the intricate link between COVID-19 and iMCD. 8 10 However, there have been no systematic reports, with sufficient sample capacity, focusing on CD patients in the COVID-19 wave and emphasizing the connection between these two inflammation-centered diseases. Therefore, to better understand of the impact of COVID-19-induced immune storms on the natural progression of iMCD, we firstly conducted a multicenter retrospective study in China.…”

“…Yamada et al reported TAFRO syndrome with a serious clinical course triggered by the second COVID-19 mRNA vaccination [ 12 ]. Hoffman et al reported a case of iMCD triggered by the second COVID-19 mRNA vaccination [ 14 ]. We encountered a case of TAFRO syndrome that developed immediately following the first COVID-19 mRNA vaccination.…”

“…Cases of kidney injury, such as IgA nephropathy [7], minimal change disease [8][9][10], and IgG4-related disease [11], following mRNA vaccination has accumulated in current research. Recently, few cases of TAFRO syndrome [12,13] and iMCD [14] developed after COVID-19 vaccination or COVID-19 infection was reported. Yamada et al reported TAFRO syndrome with a serious clinical course triggered by the second COVID-19 mRNA vaccination [12].…”
