IgE myeloma: a case presentation and a review of the literature

Abstract: In the literature, fewer than 40 cases of IgE myeloma have been described. We report the first Norwegian case, an 80-year-old man presenting with progressive weakness, dyspnea and dizziness. With the exception of hypersedimentation, routine chemistry values were within reference limits. Plasma cells were not observed in the peripheral blood. Serum protein electrophoresis showed a monoclonal protein in the gammaglobulin fraction. Immunofixation confirmed the presence of an IgE kappa monoclonal protein. A bone m… Show more

“…Testing urine for proteins is a simple way of looking for light chains; sulfosalicylic acid test rather than the dipstick is recommended because the latter often misses light chains; a negative test result makes the likelihood of Bence Jones proteinuria extremely small ( [14,19]; Robert A Kyle, personal communication). Clinical features of IgD and IgE myeloma are similar to that of other myelomas but Bence-Jones proteinuria, extramedullary involvement, lytic lesions and amyloidosis have been reported to be more frequent in IgD [26]; plasma cell leukemic presentation is common in IgE myeloma [27].…”

“…IgE myeloma is in fact similar to classical myeloma in sex distribution and age of onset. Of the known cases of IgE myeloma including the one in the current report, the male-to-female ratio was 20:18, and ages of these patients ranged from 38 to 80 years (Jako et al, 1997;Kairemo et al, 1999;Lloyd et al, 2003). IgE myeloma has no specific or typical symptoms and findings when compared with the other forms of MM.…”

“…The average survival from time of diagnosis in a review of 38 cases (Kairemo et al, 1999) was significantly shorter in patients with IgE myeloma (one year) than those with other forms of myeloma (30 months) (Yoshitake et al, 1976). Our patient is alive well and healthily with an 18-month complete remission up to now.…”

“…IgE is one of a handful of laboratory values with a logarithmic range. Monoclonal production of IgE is rarely found in plasmocytoma and has not been reported in children (17). Polyclonal increases of serum IgE in developed countries are most frequently caused by an allergen-specific IgE antibody response, as in atopic eczema/dermatitis syndrome (AEDS) (18).…”
