Image navigation as a means to expand the boundaries of fluorescence-guided surgery

Abstract: Hybrid tracers that are both radioactive and fluorescent help extend the use of fluorescence-guided surgery to deeper structures. Such hybrid tracers facilitate preoperative surgical planning using (3D) scintigraphic images and enable synchronous intraoperative radio- and fluorescence guidance. Nevertheless, we previously found that improved orientation during laparoscopic surgery remains desirable. Here we illustrate how intraoperative navigation based on optical tracking of a fluorescence endoscope may help … Show more

“…This can, in part, be realized using a hybrid tracer that contains 2 distinct signals, for example, the sentinel node (SN) tracer indocyanine green (ICG)-99m Tc-nanocolloid (1). Recently, this hybrid concept was expanded beyond the field of tracer development to hardware-based integration of radioguidance and FI modalities (4,5).…”

“…7). This approach was used to navigate towards the prostate and SNs in a patient undergoing robot-assisted prostatectomy [68], as well as in a patient with a primary parotid malignancy scheduled for the SN procedure [69]. Also crucial for SPECT/CT image-guided surgical navigation, in addition to the creation of a mixed reality environment, is realtime visualisation or guidance.…”

“…As well as allowing real-time acquisition of SPECT scans in the operating room, the system can also load preoperatively acquired SPECT/CT data. It has been shown that this approach can also be used to navigate tools other than the gamma probe, e.g., a fluorescence laparoscope, towards radioactive hotspots [76]. Fluoresence imaging allows errors within the 1-cm range (due to organ movement or deformation for example) to be compensated for [76].…”
