Immediate and Durable Clinical Improvement in the Non-Operated Hand after Contralateral Surgery for Patients with Bilateral Carpal Tunnel Syndrome

Abstract: Background Little is known about clinical improvement in the non-operated hand after unilateral surgery for patients who present with bilateral carpal tunnel syndrome (CTS). In this prospective study of patients with bilateral CTS, we evaluated the clinical effects on the non-operated hand following unilateral contralateral carpal tunnel surgical release. Material and Methods During a consecutive period of 22 months, 69 patients with bilateral CTS underwent unilateral open carpal tunnel release. Bilateral subj… Show more

“…Carpal tunnel syndrome (CTS) is the most common compressive entrapment neuropathy caused by the compression of the median nerve at a space around the wrist known as the carpal tunnel. The prevalence of CTS in the United Kingdom is 70-160 cases per 1,000 subjects [ 1 - 3 ]. Many risk factors have been correlated with the development of this compressive neuropathy.…”

“…49,50 The results of surgery on NH are conflicting in literature. Unno et al 51 evaluated patients with bilateral CTS operating on only one hand and reported improved hand NH both immediately postoperatively and at follow-up for 6 months after surgery, regardless of the severity of NH impairment. 51 According to Agrawal and Southern, 52 the improvement of NH hand in bilateral CTS cases showed spontaneous recovery in 37% of cases after 6 months of follow-up period.…”

“…It is noteworthy that the published literature additionally reports that improvement of the contralateral hand was found in 74–84% of cases, in objective and subjective evaluations with stable beneficial effects at 180 days [ 27 ]. This report showed that gender, age, professional status, duration of preoperative symptoms, and electrophysiological severity were not predictive.…”
