Background: Four administration schedules of immunotherapy have been reported: conventional, cluster, rush and ultra-rush. Objectives: To evaluate the safety and the clinical advantage of using standardized modified allergen extracts in an ultra-rush protocol without premedication and/or hospitalization. Material and Methods: One thousand and sixty-eight patients with rhinoconjunctivitis and/or asthma sensitized to mites and/or pollen were included in a prospective observational study. Patients received a therapeutic vaccine containing depigmented and glutaraldehyde-modified extracts (mites and/or pollens) adsorbed onto alum prescribed by a specialist. The schedule of administration consisted of injecting 0.2 and 0.3 ml of the vial of maximum concentration during the first day of immunotherapy, separated by a time interval of 30 min. All patients reached the maximum dose (0.5 ml) after 2 injections. Tolerance was assessed by recording all side reactions related to immunotherapy, classified according to the criteria of the EAACI. Results: The total number of injections was 2,136. All patients reached the maximum established dose on the 1st day. No premedication was used. Seven clinically relevant local reactions were recorded. The systemic reactions were 5 grade-1 (2 immediate and 3 delayed) and 3 delayed grade-2 reactions. Conclusions: The therapeutic vaccines containing chemically modified extracts can be administered using an alternative ultra-rush schedule in an immunotherapy unit, reaching the maximum dose on the 1st day with 2 injections, without the need of premedication and/or hospitalization.

Background: Polymerised allergenic extracts (allergoids) are commonly used in allergen immunotherapy. Clinical efficacy and safety of these extracts have been demonstrated. Recently, allergen sequences have been identified by mass spectrometry in depigmented and polymerised (Dpg-Pol) extracts. The objectives of this study were to investigate the presence of allergens in Dpg-Pol extracts of house dust mite and to analyze the immunological changes induced by these extracts in asthmatic patients enrolled in a double-blind, placebo-controlled study. Methods: Dpg-Pol extracts were manufactured and vaccines with a composition of 50% Dermatophagoides pteronyssinus and 50% D. farinae (100 HEPL/ml) were prepared. Allergen composition was analyzed by mass spectrometry. Patients with asthma and rhinoconjunctivitis were treated in a 1-year, double-blind, placebo-controlled, parallel-group study with 6 up-dosing and monthly maintenance injections. Specific IgE and IgG4 titres to D. pteronyssinus, Der p 1 and Der p 2 were measured in patients’ sera using the CAP system and direct ELISA experiments. Results: Sequences from the major allergens Der p 1 and Der p 2 and from other allergens were identified in native and Dpg-Pol extracts. There was a statistically significant increase in specific IgG4, a decrease in the ratio of IgE/IgG4 to D. pteronyssinus and a significant increase in specific IgG4 to Der p 1 and Der p 2 in the patients allotted to active treatment. Conclusions: The detection of allergen sequences suggests preservation of major and minor allergens in Dpg-Pol allergoids from house dust mites. Efficacy in asthma treatment and the increase in specific IgG4 seem to be associated with the presence of major allergens in Dpg-Pol allergen extracts.

Como sucede en otras partes del mundo, la prevalencia de asma y rinitis alérgica en Colombia está en aumento. Se ha establecido que la inmunoterapia subcutánea con alérgenos es eficaz a largo plazo en pacientes con rinitis alérgica y asma sensibilizados a Dermophagoides. Objetivo. Proveer evidencia sobre los cambios relacionados con la calidad de vida inducidos por la inmunoterapia subcutánea en sujetos con alergia respiratoria. Materiales y métodos. Se seleccionaron 76 sujetos con diagnóstico de alergia respiratoria con sensibilización a Dermatophagoides farinae y Dermatophagoides pteronyssinus. Para la evaluación de la calidad de vida se emplearon los instrumentos Kidscreen-27 y SF-36 (Short form 36). Estos instrumentos se aplicaron en dos ocasiones: durante la primera visita, en la cual se iniciaba la inmunoterapia subcutánea, y un año después de haberse iniciado el tratamiento.Resultados. Al año de estar recibiendo la inmunoterapia, los 22 sujetos que completaron el estudio presentaron cambios positivos en términos de calidad de vida. En los niños, el principal cambio se presentó en el dominio del 'entorno escolar' mientras que en los adultos fue en el de la 'función física'. Discusión. Se evaluaron por primera vez en Colombia los beneficios inducidos por la inmunoterapia subcutánea para ácaros de polvo en la calidad de vida de sujetos con rinitis alérgica y asma mediante los cuestionarios Kidscreen-27 y SF-36. Los resultados proveen evidencia de que la inmunoterapia subcutánea influye positivamente en la calidad de vida en sujetos con rinitis asmática y asma sensibilizados a los ácaros de polvo. Abstract: Results of this study provided evidence that IT with depigmented, glutaraldehyde-modified allergen extracts was well-tolerated and added beneficial effects to AR treatment in pollen allergic patients eliciting an improvement in QOL enough to justify a change in the patient's treatment.
