Impact of comorbidities constituting the hematopoietic cell transplant (HCT)‐comorbidity index on the outcome of patients undergoing allogeneic HCT for acute myeloid leukemia

Abstract: Among the comorbidities that compose the HCT-CI, diabetes and cardiovascular comorbidity independently predict NRM in patients undergoing allogeneic HCT for AML. This information should be taken into consideration regarding post-transplant monitoring and care.

“…The recent EBMT cohort contains a higher frequency of dose-reduced conditioning (52% vs. 28% original HCT-CI cohort), unrelated donors (64% vs. 42%), and older patients (median age 53.3 vs. 44.8 years) as compared to the original HCT-CI cohort. Over the last few decades, considerable progress has been made regarding the reduction of NRM, which is most likely due to progress in intensive care medicine, as well as to improved management of GVHD, conditioning regimens, and prevention/treatment of infectious complications in alloSCT recipients [ 5 , 9 , 11 13 ].…”

“…The Framingham general CV risk score has also shown increased CV risk in male survivors [96]. Interestingly, diabetes and CV comorbidity as derived by the HCT comorbidity index independently predicted nonrelapse mortality in a recent study [97].…”

“…Similarly, Fein et al demonstrated that certain comorbidities were associated with an increased risk of TRM when particular conditioning regimens were chosen, while the impact of a high HCT‐CI score was also regimen specific . Another recent study in patients with acute myeloid leukemia showed that particular pretransplant comorbidities had a disproportionate effect on outcomes compared to what would have been expected based on the HCT‐CI score . These reports support that the presence of certain risk factors may lead to regimen‐specific toxicity, and therefore early recognition of these comorbidities may allow providers to tailor conditioning regimens for individual patients in the hopes of improving outcomes.…”
