Improving school water, sanitation and hygiene (WASH) conditions has been shown to be effective in reducing pupil absence and illness. However, the benefit of sanitation and hygiene promotion improvements at school may depend on the conditions of the latrines and availability of consumables. We employed a three-arm, cluster-randomized trial to determine if a low-cost, policy-relevant, environmental-level latrine cleaning intervention could a) improve latrine cleanliness, b) increase latrine use, and c) reduce absenteeism. We assessed absence via periodical roll-call among 17,564 pupils in 60 schools that previously received WASH Improvements as part of the SWASH+ project. Latrine conditions and use were also assessed using structured observation. Latrine cleanliness increased significantly during the post-intervention period among schools receiving the latrine-cleaning package compared to controls. Handwashing with soap increased as well in intervention schools relative to controls. We found no difference in latrine use and absence across arms. The additive impact of cleaning may not have been strong enough to impact absence above and beyond reductions attributable to the original WASH infrastructure improvements and basic hygiene education the schools previously received. Improving latrine conditions is important for the dignity and well being of pupils, and investments and strategies are necessary to ensure school sanitation environments are clean and pupil-friendly.

Background: Diarrhea and pneumonia accounts for an estimated 40% of all child deaths around the world each year. Handwashing with soap could protect about 1 out of every 3 young children who get sick with diarrhea and almost 1 out of 6 young children with respiratory infections like pneumonia. Although people around the world clean their hands with water, very few use soap to wash their hands. Washing hands with soap removes germs much more effectively. This study was undertaken to determine the level of knowledge and practice regarding proper handwashing among elementary school children. Methods: A descriptive study was conducted from August to October 2014 in Jatinangor using primary data from respondents who are 5th graders from four schools. A total of 49 males and 50 females were chosen using cluster random sampling. Schoolchildren who were present, able and well were given questionnaires after written informed consent was obtained from the schoolchildren and their guardians. Then, respondents demonstrated hand-washing techniques which were evaluated using a checklist. Results: Overall, 52% of the schoolchildren had moderate level of hand-washing knowledge and 66% had bad handwashing practices. Conclusions: Despite having a moderate level of handwashing knowledge, the majority still had poor handwashing practices. Handwashing programs should be carried out extensively to improve knowledge and practice.

Introdução. Afastamentos associados com as doenças infecciosas estão entre as principais causas de absenteísmo de escolares. A legislação atual não dispõe sobre os conteúdos ou as disciplinas que deveriam abordar a importância da higiene das mãos para se evitar a transmissão de doenças infecciosas, tornando necessário investigar como as escolas lidam com o assunto. Objetivo. Expor algumas reflexões sobre os Parâmetros Normativos e Matrizes Curriculares, especificamente o tema transversal relacionado à saúde e contextualizar as condições de infraestrutura para higiene nas escolas do ensino fundamental do Município de Uberaba, MG. Material e métodos. Primeiramente, realizou-se uma análise documental seguida de uma revisão integrativa da literatura. Posteriormente, conduziu-se um estudo observacional-seccional (diagnóstico situacional). Para a busca foram utilizadas as bases de dados periódicos CAPES, SciELO, LILACS, MEDLINE, PubMed, CINAHL e Ministério da Educação. A observação estruturada ocorreu em visitas agendadas nas escolas, e foi utilizado um instrumento de observação validado. Os dados foram submetidos à análise descritiva. Resultados. A educação é garantida pela Constituição de 1.988 a todos os brasileiros, demonstrando uma preocupação do Estado em manter as crianças nas escolas. Entretanto, a legislação relacionada com o ensino dos temas transversais e questões de saúde surgiu somente dezenove anos mais tarde. Atualmente, as matrizes escolares tratam do tema higiene nos conteúdos de Ciências e Educação Física, porém esse assunto também foi encontrado em Geografia e História. Pesquisas internacionais mostram que as crianças têm baixa adesão na higienização das mãos, o que pode favorecer o surgimento de doenças infecciosas e parasitárias. As escolas do município de Uberaba que foram visitadas possuíam infraestrutura adequada, porém os insumos para a higiene das mãos não eram, em sua maioria, disponibilizados nos banheiros. Conclusão. A legislação mostra a preocupação do Estado com a Educação e a Saúde dos escolares, entretanto, parece haver um hiato na aplicabilidade entre essas leis e a prática. A revisão da literatura internacional revela que as intervenções educativas no ensino fundamental acarretam redução do absenteísmo por infecções. Todavia, as publicações nacionais não permitiram estabelecer tal associação. As escolas estudadas oferecem a infraestrutura física para a higiene dos escolares, entretanto, serão necessários esforços adicionais para conscientizar profissionais e escolares para a disponibilização e uso adequado dos insumos requeridos para a higiene das mãos.
