Impact of the mobilization regimen and the harvesting technique on the granulocyte yield in healthy donors for granulocyte transfusion therapy

Abstract: G-CSF plus dexamethasone produces higher granulocyte yields than G-CSF alone. Granulocyte collection from PBSC donors appears to be a rational strategy, since it produces high granulocyte yields when the related patients are at a high risk for infection and reduces difficulties in finding granulocyte donors. HES should be used in apheresis procedures.

“…COBE Spectra MNC has been used for several decades and its technology has been described in many publications 21,21,23‐25. For granulocyte collection, the PMN protocol was used . Therefore, we will refer to granulocyte collections with COBE Spectra MNC with the PMN protocol as COBE Spectra PMN throughout the remainder of the article.…”

“…As to the different apheresis systems available for granulocyte extraction, not many data have been reported . A current standard apheresis device for granulocyte apheresis is the COBE Spectra MNC system and tubing set operating with the PMN collection protocol. In one sitting, during which approximately 6 to 7 L of blood is processed, a product containing what is considered one unit of granulocytes according to the German transfusion guidelines, that is, 1 × 10 10 granulocytes, can be generated.…”

“…Clearly, this was easier to achieve in a pediatric as opposed to an adult population. The ANC increment correlated with the granulocyte dose, reported in previous studies, [2][3][4]11,13,18 and emphasizes the importance of achieving an optimal granulocyte collection.…”
