Implementación de las guías de práctica clínica sobre ictus isquémico agudo en atención especializada

Abstract: In our opinion, secondary prevention of acute cerebrovascular disease could be improved, mainly by increasing the percentage of patients treated with antiplatelet or anticoagulant drugs (depending on aetiology), increasing prescription of statins, and improving blood pressure control.

“…Upon discharge from hospital following an IS episode, between 38% and 76% of patients were receiving LLT [ 39 , 40 , 49 ]. In addition, one observational study with 203 patients admitted for medium- to long-term stays in one hospital between 2009 and 2010 found that only 20% of the total number of post-event patients received the dosage recommended by current guidelines [ 54 ].…”

“…With regards to attained cholesterol levels, 3 studies were found [ 39 41 ], reporting figures on LDLc control of <100 mg/dl between 25% and 33%. In two additional studies [ 54 , 61 ] in which targets were set at Total Cholesterol (TC) < 175 mg/dl, control rates were between 43% and 77% (Table 2 ; Fig. 2 ).…”

“…Regarding the main vascular risk factors, the prevalences of arterial hypertension (60%), diabetes (26.5%), and prior stroke (25%) cited by the Hospital de Torrecárdenas group are lower than in our study (68%, 40%, and 34%, respectively). 1 As stated before, the reason for this discrepancy is probably that patients seen in the rehabilitation unit of an acute care hospital tend to be younger, and would therefore show a lower prevalence of the listed factors. Nevertheless, it is interesting that the reported prevalence of atrial fibrillation (18.3%) was similar to that in our study (20.2%) 1 even though prevalence of this condition increases with age.…”

“…1 The letter cited above describes the analysis of a sample of 173 patients diagnosed with acute cerebrovascular disease and attended by the rehabilitation unit at Hospital Torrecárdenas in Almería. On average, patients in that sample were younger than those in our own group (mean of 70 years vs 75 years).…”
