Implementation of a cloud-based referral platform in ophthalmology: making telemedicine services a reality in eye care

Abstract: BackgroundHospital Eye Services (HES) in the UK face an increasing number of optometric referrals driven by progress in retinal imaging. The National Health Service (NHS) published a 10-year strategy (NHS Long-Term Plan) to transform services to meet this challenge. In this study, we implemented a cloud-based referral platform to improve communication between optometrists and ophthalmologists.MethodsRetrospective cohort study conducted at Moorfields Eye Hospital, Croydon (NHS Foundation Trust, London, UK). “…Over the past two decades, therefore, an important foundation has been laid for the efficient implementation of integrated digital ophthalmic care [517,518]. Currently, the 'store-and-forward' model of teleophthalmology is now being used, where images are taken at a different time and place than are then assessed by a trained grader, and this is best illustrated by the UK National DESP [275].…”

“…5 A recent innovation at MEH has focused on allowing store-and-forward methodologies pioneered in these hospital-based virtual clinics to extend to community optometry data collection. 6 Owing to the dependence on in-person examination using slit-lamp biomicroscopy, however, video consultations have traditionally been thought of as a niche technology in ophthalmology. The ongoing COVID-19 outbreak has provided a unique opportunity to challenge these preconceptions.…”

“…56 Elsewhere, the use of tablets as a means of sharing clinical data between a patient's eyecare team has also been explored. 61 With respect to traditional PROMs, it is worth stressing that the present tests are not intended as a like-for-like replacement. Asking patients directly remains the best way to ascertain how someone feels about their condition, and PROMs, when used well, 62 have been shown capable of providing important insights into the everyday difficulties that patients face.…”
