Implementation of a complex intervention to improve participation in older people with joint contractures living in nursing homes: a process evaluation of a cluster-randomised pilot trial

Abstract: Background: Joint contractures in frail older people are associated with serious restrictions in participation. We developed the Participation Enabling CAre in Nursing (PECAN) intervention, a complex intervention to enable nurses to promote participation in nursing home residents with joint contractures. The aim of this study was to examine the feasibility of the implementation strategy and to identify enablers and barriers for a successful implementation. Methods: The implementation of PECAN was investigated … Show more

“…Several papers suggested it may be more appropriate to design research interventions with high staff turnover and a changing context in mind [ 106 , 110 ]. The importance of purposefully including flexibility within the trial design to tailor the research process to a CH’s individual context settings was also discussed as an important enabler to successful implementation processes [ 107 , 108 , 112 ].…”

“…As reliance upon change agents is common in many other intervention settings (e.g. hospitals, police custody suites, nursing homes [ 15 17 ]), our findings have wider relevance beyond school-based research.…”

“…4,5 Given this high prevalence rate of joint contracture among elderly people, recent research using the International Classification of Functioning, Disability and Health (ICF) as a framework indicates that joint contracture is associated with numerous limitations of functioning, such as mobility, self-care, sensory function and pain, domestic life and community, and social and civic life. 6 This is, therefore, a socially important disease in an aging society. However, its pathophysiology is unclear, and it responds very poorly to currently available treatments, primarily physical therapy.…”

“…Indeed, the prevalence of joint contracture in residents of nursing homes is reported to be 55%–75% 4,5 .
