Impression Cytology on Conjunctiva and Cornea in Dry Eye Patients Establishes a Correlation between Squamous Metaplasia and Dry Eye Clinical Severity

Abstract: A morphologic and morphometric analysis of the ocular surface from patients with dry eye obtained by impression cytology led us to draft a new grading system containing one normal level and five levels of squamous metaplasia. This new grading system is based on a significant decrease in the number of goblet cells with less periodic acid-Schiff-hematoxylin-positive staining, an increase in nonsecretory cell size, more marked cell separation, a lower N:C ratio, and an increase in nuclear alterations. The clinica… Show more

“…8 As noted, these changes occur mainly in dry eye-related conditions. [14][15][16][17] In our patients, we also found the presence of inflammatory cells. It is known that impression cytology has made an important contribution to the understanding of dry eye as an immune-based inflammatory condition.…”

“…In addition, CMC has been shown to stimulate corneal epithelial cell migration [8] and a significant improvement in goblet cell density has been observed after one month of CMC treatment [38], further enhancing the mucus layer of the tear film. A reduction in goblet cell density is likely to be associated with ocular surface inflammation and loss of vascularisation [39] and appears to be an early sign of an abnormality of epithelial differentiation (also examined using impression cytology) [40,41].…”
