Purpose Cystic fibrosis (CF) is the most frequent lethal autosomal recessive hereditary disorder. The disease affects all secretory epithelia, including the eye, and belongs to the group of ocular surface epithelial diseases, termed keratoconjunctivitis sicca or dry eye syndrome. The aim of this study was to evaluate goblet cell population and conjunctival epithelial morphology in patients with CF. Methods A total of 20 CF patients and 20 controls underwent conjunctival impression cytology.Results Impression cytology showed conjunctival squamous metaplasia and goblet cell loss in patients with CF. Conclusion Reduced goblet cell numbers and squamous metaplasia may be indicative of a higher degree of epithelial damage of conjunctival epithelial cells in CF patients, and the presence of neutrophils is a strong sign for an inflammatory background of this disease. In view of the simple, noninvasive nature of impression cytology, this technique may prove to be an important tool for the diagnosis and monitoring of dry eye changes in CF patients.

PurposeTo assess dry eye treatment with four preservative-free dry eye artificial tear treatments to facilitate evidence-based prescribing. Methods ResultsThe impact of each dry eye treatment on ocular signs and symptoms was similar, however OSDI (p=0.002), LIPCOF (p=0.014) and conjunctival staining (p<0.001) significantly improved from baseline. Clinitas Soothe and Hyabak were preferred by 34%/30% of participants, but only subjective comparison with the other drops influenced this choice. TheraTears was preferred (by 24%) by those with a lower baseline tear volume (p=0.01) and Tears Again (by 12%) by those with a thinner baseline lipid layer (p=0.04). The treatment that afforded the greatest improvement in clinical signs did not consistently match each individual's preferred treatment. ConclusionsIf prescribed to a general dry eye population, the artificial tears performed similarly, improving symptoms and conjunctival signs. However, osmolarity balanced artificial tears were the preferred treatment in individuals with low baseline tear volume and lipisomal spray for individuals with a baseline lipid layer deficiency. RESUMENObjetivos: Determinar si existe relación entre el grado de metaplasia escamosa de la superficie corneal mediante citología de impresión y la gravedad clínica en pacientes con insuficiencia limbal. Métodos: Hemos estudiado mediante citología de impresión 98 ojos de pacientes con insuficiencia limbal que fueron divididos en cuatro grupos según la gravedad clínica: 14 ojos no presentaban sintomatología alguna, 34 ojos presentaron un grado leve de insuficiencia limbal, 28 ojos un grado moderado y 22 ojos una insuficiencia limbal grave. A todos los pacientes se les realizó una citología corneal en la que se estudió el tamaño celular, tamaño nuclear, alteraciones nucleares y relación núcleo-citoplasma (N:C), así como la presencia de células caliciformes. Resultados: Los pacientes con insuficiencia limbal asintomática presentaron un tamaño celular medio de 477 DE 140 µm 2 , con una relación N:C de 1:5,25 DE 1,5. Los pacientes con insuficiencia limbal leve presentaron un tamaño celular medio de 764 DE 122,6 µm 2 y una relación N:C de 1:8,2 DE 1,4. Methods: We studied 98 eyes of patients with limbal deficiency by impression cytology. These patients were divided into four groups in relation to their clinical severity: 14 eyes had no symptoms, 34 eyes showed a mild grade of limbal deficiency, 28 eyes had a moderate grade and 22 eyes had severe limbal deficiency. Corneal cytology was performed in each patient. Cellular size, nuclear size, nuclear changes and the nuclear-cytoplasmic (N:C) ratio were defined in corneal epithelial cells, in addition to evaluation for the presence of goblet cells in the corneal epithelium. Results: In patients with limbal deficiency without symptoms, we found that the cellular size was 477 (SD140) µm 2 and the N:C ratio was 1:5.25 (SD1.5).
