Improvement of Pituitary Function after Surgical Decompression for Pituitary Tumor Apoplexy*

Abstract: Hypopituitarism is a major manifestation of pituitary adenoma apoplexy. We postulated that the acute deterioration in pituitary function may be caused by compression of portal vessels and the pituitary stalk, secondary to a sudden increase in intrasellar contents. If this were the case, one would predict improvement in pituitary function to occur after surgical decompression. We studied pituitary function in eight patients presenting with the clinical syndrome of pituitary adenoma apoplexy before and on multip… Show more

“…Traditionally, most patients with apoplexy were treated surgically after this initial stabilization. 3,8,13,16,31 However, more recent series have repeatedly reported good outcomes in selected patients treated conservatively with medical therapy only. 5,9,13,17,21,23,25,31 In our study, 70% of the patients were treated with early resection, but the rest were treated with a delayed, planned surgical intervention (9%) or solely with medical treatment (21%).…”

“…Many authors have advocated surgical decompression in patients with prominent visual disturbance or neurologic deficits. Surgery may also improve pituitary function [18]. Recently, however, many cases showing spontaneous recovery with satisfactory results after conservative treatment have been reported [7,9,19].…”

“…Significant improvement of the pituitary dysfunction has been reported in one study (86), in which the authors concluded that early surgery allows reversal of the hypopituitarism attributed to the acute compression of the portal vessels by the enlarging pituitary mass. This favourable outcome was not replicated by others (10).…”

“…Hypopituitarism is another major manifestation of PA, probably secondary to the acute increase in the intrasellar pressure compressing the portal circulation, the pituitary stalk and the pituitary gland itself (86). Hyponatraemia can occur secondary to hypopituitarism or, rarely, to inappropriate ADH secretion (7,87).…”
