Improving Information Transfer during Transitions of Care via Standardized Handoffs

Abstract: Multiple studies have shown the detrimental effect of miscommunication during transitions of care. The aim of this study is to determine whether a certain method of “sign-out” can improve information transmission and thereby reduce medical errors.
