Improving the management of dyspnea in the community using rapid learning approaches

Abstract: Patients with chronic pulmonary disease often suffer from breathlessness or dyspnea. Traditional evidence generation techniques to expand upon current treatment paradigms are limited by the significant delay between study initiation and clinical implementation of findings. Rapid learning health care is a novel approach to health care delivery that relies on intelligent and continuous integration of clinical and research data sets to deliver personalized medicine using the most current evidence available. “…Others have recently promoted the concept of a rapid learning model, similar to the "plan, do, study, act" cycle promoted by the Institute for Healthcare Improvement, 37 to disseminate new information from clinical trials and consensus statements from professional societies about palliation of dyspnea. 38 To address the many knowledge gaps regarding patient and caregiver perspectives, we are collecting, in an ongoing clinical trial, both quantitative and qualitative assessments of patient and caregiver experiences before and for six months after opioids are initiated for refractory dyspnea in advanced COPD.…”

“…• Start small and learn quickly: Significant opportunity lies in the principles of quality improvement and rapid learning. 47 Utilizing the Institute for Healthcare Improvement approach with a set aim, clear metrics, and rapid iterative cycles is feasible and necessary to create new and successful models for integration. • Leverage primary and specialist palliative care teams: Understanding the role of primary teams for the provision of palliative care is critical for integrating palliative care into the continuum of cancer care.…”
