Background-The present study was designed to examine the influence of suicidality on relapse in alcohol-dependent patients. Specifically, a lifetime suicide attempt at baseline was used to predict relapse in the year after treatment. Also, the unique contribution of impulsive suicide attempts was examined.Methods-A total of 154 patients with alcohol dependence, consecutively admitted to four addiction treatment facilities in Warsaw, Poland participated in the study. Of the 154 eligible patients, 118 (76.6%) completed a standardized follow-up assessment at 12 months.Results-Previous suicide attempts were common in adults treated for alcohol dependence with 43% patients in the present sample reporting an attempt at some point during their lifetime. Additionally, more than 62% of those with a lifetime suicide attempt reported making an impulsive attempt. Lifetime suicide attempts were not associated with post-treatment relapse (chisquare=2.37, df=1, p=0.124). However, impulsive suicide attempts strongly predicted relapse (OR=2.81, 95%C.I. = 1.13-6.95, p=0.026) and time to relapse (OR=2.10, 95% C.I. = 1.18-3.74, p=0.012) even after adjusting for other measures of baseline psychopathology, depression, impulsivity, hopelessness and alcohol use severity.Conclusions-This study is the first to document the relationship between pre-treatment impulsive suicide attempts and higher likelihood of post-treatment relapse in alcohol-dependent patents. Clinicians should routinely conduct an assessment for previous suicide attempts in patients with alcohol use disorders, and when impulsive suicidality is reported, they should recognize the increased risk for relapse and formulate their patients' treatment plans accordingly with the goals of reducing both alcoholic relapse and suicide rates.

Prevalence and risk factors of suicide in a rural area of Shandong province in mainland China: a case control study

Objective: Suicide is a major public health problem in China, particularly in rural areas. This study aimed to examine the rate and the risk factors for suicide in a rural area of China. Methods: The mortality data of two counties in Shandong Province from January 1st, 2004 to December 31st 2005 were used. Death information was collected via face-to-face interview with informant(s). Psychological autopsy was conducted on 137 completed suicides and 137 matched controls died from other illnesses. Chisquare/Fisher exact and multiple logistic regression were used for bivariate analyses and predictor analyses, respectively. Results: The suicide rate was 20.90 per 100,000 persons per year with almost equal rates of males and females. Among those with the age over 40 years old, the suicide rate in males was significantly higher than that in females. The peak of suicide fell in the age group of 70-79 years for males and the age group of ≥ 80 years for females. The most commonly used method for suicide was pesticide ingestion (76.7%). Poor interpersonal relationship (OR=12.79), childhood adversities (OR=7.26), a history of mental illness (OR=5.95), adverse life event(s) within 1 year prior to death (OR=3.19), and unstable extravert personality traits (OR=6.12) were independent risk factors for suicide. Male gender positively interacted with majority of these risk factors, especially with a history of mental illness (OR=17.89). Advanced age positively interacted with all independent risk factors, with a history of mental illness having the largest OR of 1.51 as every 10 year of age advancement. In bivariate analyses, a history of previous suicide attempt (OR=13.06), a family history of suicide (OR=7.08), and unstable introvert personality trait (OR=1.83) were also associated with increased risk for suicide; and stable extravert personality trait (OR=0.21) was associated with decreased risk for suicide. Conclusions: In this rural population of mainland China, the risk for suicide was almost equal in both males and females with the elderly having the highest risk. The positive interactions of advanced age and male gender with independent risk factors suggest that suicide prevention strategies targeting specific groups with different risk factors are essential.

Introduction: The issue of suicidal risk ,and suicide in general, represents one of the biggest challenges in psychiatric evaluation. This issue is covered by an extensive scientific work with the purpose of creating the most precise evaluation of suicide and suicide risk. Aim: The main goal of this research is to validate psychopatological constructs of a personality isolated by MMPI-a and HAMD in relation with measured suicidal rate among the subjects in question. Also, goal of this research is to explore possible connecitons beetween standard clinical and sociodemographic variables with rate of suicidality. Material and methods: The sample group consists of patients that are based on the stationary ward of a psychiatric clinic, diagnosed with F32 and F33. All the data was collected by interview, questionnaire and official medical documentation. The prediction variables are presented by the score on MMPI-202 and HAM-D, and standard clinical and sociodemographic variables. Criteria variables represent the score on SBQ-R, as a measure of suicidal tendencies, and a category variable which marks the presence of suicide attempts in the past. During the data analysis, the following methods were used: descriptive statistics, bivariate correlation, regression analysis, comparisment of arithmetic mean and canonical discriminant analysis. Results: The research showed a positive and significant correlation between suicidal tendencies and the scales of: depression, paranoia, schizophrenia, anxiety, social isolation, psychoastenia and the hospitalization count. The only one reliable predictor of suicidalty is depression. Conclusion:The research concludes that the psychopathological constructs, isolated by MMPI, are connected with suicidal tendencies. However, the conclusion is that further research is necessary in terms of determining the quality and way of the connections that are achieved between the test and the suicidality.
