Incidence, predictors, and treatment options of critical limb ischaemia after use of collagen plug-based vascular closure devices

Abstract: CLI after use of a collagen plug-based VCD is rare. Peripheral artery disease was identified as an independent predictor of CLI. Interventional cardiologists should be aware of potentially high-risk patients and complications after use of a VCD to provide prompt and adequate therapy.

“…The increasing number of annual endovascular procedures worldwide results in the widespread use of VCDs, which is also accompanied by an elevated incidence of related complications. As one of the most severe complications, acute lower limb ischemia has been described after the use of all types of VCDs , with a prevalence of 0.1%–4.2% after use of the Angio‐seal TM closure device . However, in the absence of clinical studies with large sample sizes, the optimal treatment strategy for patients with Angio‐seal TM ‐related acute limb ischemia remains in dispute.…”

“…Angio‐Seal™ (St. Jude Medical, The United States) is one of the most commonly used VCDs, which deposits a small collagen plug into the puncture channel to mechanically seal the puncture defect . The most serious complication related to the application of the Angio‐Seal™ closure device is acute limb ischemia due to femoral artery stenosis and occlusion, which requires revascularization treatment primarily to restore blood flow through lower limb arteries and to relieve ischemic symptoms.…”
