BackgroundThe recent finding that aortic pulse wave velocity (aPWV) is increased in patients with inflammatory bowel disease may explain why the cardiovascular risk is increased despite the low prevalence of traditional cardiovascular risk factors. We aimed to test whether inflammation is associated with aortic stiffening in this setting after adjustment for major confounders and to perform subgroup analyses.Methods and ResultsA systematic literature search for aPWV in inflammatory bowel disease was performed using PubMed, Scopus, Web of Science, and Google Scholar databases (last accessed May 7, 2017). Inclusion criterion was peer‐reviewed publications on clinical studies reporting original data. This study followed the Preferred Reporting Items for Systematic Review and Meta‐Analyses of individual participant data 2015 guidelines. Data were provided for 4 cohorts in 3 countries (151 participants with ulcerative colitis, 159 with Crohn's disease, and 227 control patients). Using aPWV, cohort‐specific z scores were calculated after loge‐transform and combined in meta‐analysis to form pooled effects using a random‐effects model. Compared with controls, aPWV was increased in patients with Crohn's disease (mean difference 0.78 z score; 95% confidence interval, 0.56–1.00 z score [P<0.001]) and ulcerative colitis (mean difference 0.75 z score; 95% confidence interval, 0.52–0.97 z score [P<0.001]). In an outlier‐robust multivariate linear regression model adjusted for prespecified confounders, aPWV was associated with disease duration (years, β=0.05 z score; 95% confidence interval, 0.02–0.08 z score [P<0.001]) and white blood cell count (billion cells/L, β=0.07 z score; 95% confidence interval, 0.02–0.11 z score [P=0.002]) but not with markers of acute inflammation (C‐reactive protein and erythrocyte sedimentation rate), cardiovascular risk factors, and therapy.ConclusionsThe increased aPWV reported in patients with inflammatory bowel disease is associated with inflammation.Clinical Trial Registration URL: http://www.crd.york.ac.uk. We evaluated the differences in major markers of cardiovascular (CV) risk between inflammatory bowel diseases (IBDs) and controls by a systematic review and a meta-analysis. We searched PubMed, EMBASE, and Cochrane databases for literature comparing CV risk markers in IBDs and controls. The overall mean carotid intima-media thickness (CIMT), flow-mediated dilation (FMD%), and carotid-femoral pulse wave velocity (cfPWV) difference between patients with IBDs and control groups were calculated. Twenty-eight studies were included in the meta-analysis, including 16 studies with data on CIMT, 7 studies reporting FMD%, and 9 studies on cfPWV. Compared to controls, patients with IBDs showed significantly higher CIMT (standardized mean difference [ SMD]: 0.534 mm; 95% confidence interval [CI], 0.230 to 0.838; P = .001), significantly lower FMD% ( SMD, -0.721%; 95% CI, -1.020 to -0.421; P < .0001), and significantly increased cfPWV ( SMD, 0.849; 95% CI, 0.589 to 1.110; P < .0001). When analyzing subgroups with ulcerative colitis and Crohn disease (CD), all results were still significant except CIMT in CD. Our findings support the current evidence for an elevated CV burden in patients with IBD and support the clinical utility of markers of subclinical atherosclerosis in the management of these patients.

Introduction: Ulcerative colitis is a chronic inflammatory disease which reduces the quality of life and patients’ productivity. People in different ages could be affected since its complications such as colon perforation and cancer are common. Oxidative stress is reported to create an imbalance between reactive oxygen species (ROS) and antioxidant defense systems which occurs in inflammatory tissues and predisposing malignant tumors. Objectives: We aimed to evaluate the effects of omega-3 on inflammatory, antioxidant, and oxidative markers of patients with ulcerative colitis. Patients and Methods: Seventy elderly patients with ulcerative colitis referring to Rasoul-e-Akram hospital (Iran University of Medical Sciences) with mild or moderate phase of the disease were investigated. Patients were undergoing their protocol treatment. Patients were randomly assigned to the block randomization assignment, including 35 patients (routine + omega-3) or control (routine + placebo). Based on a researcher-made checklist, the severity of the disease, as well as the oxidant, antioxidant, and inflammatory factors of the patients were determined before and after 2 months of treatment. Finally, the changes in these factors were compared between groups. Results: Among the 70 participants, 35 were in the control and 35 in the intervention group. All subjects were present during the whole period of the study, so the attrition rate was 0%. No significant difference was observed between the mean of BMI (body mass index) as kg/m2, waist circumference (cm), systolic pressure (mm Hg), and diastolic pressure (mm Hg) in both groups before and after the intervention. The measure of optimal changes in serum levels of MDA (malondialdehyde oxidant), AGEs (advanced glycation end-products), oxidized LDL (oxidized low density lipoprotein), TNF-α (tumor necrosis factor-alpha) and the improvement of antioxidant catalase copper markers were significantly higher in the intervention group, compared to the control one. Conclusion: Administration of omega-3 as an auxiliary component in the treatment of patients significantly reduced the levels of inflammatory and oxidative markers and a significant increase in antioxidant markers in the serum and improved blood pressure in patients with ulcerative colitis.
