Systematic Review and Meta-Analysis of Circulating S100B Blood Levels in Schizophrenia

S100B is a calcium-binding protein secreted in central nervous system from astrocytes and other glia cells. High blood S100B levels have been linked to brain damage and psychiatric disorders. S100B levels have been reported to be higher in schizophrenics than healthy controls. To quantify the relationship between S100B blood levels and schizophrenia a systematic literature review of case-control studies published on this topic within July 3rd 2014 was carried out using three bibliographic databases: Medline, Scopus and Web of Science. Studies reporting mean and standard deviation of S100B blood levels both in cases and controls were included in the meta-analysis. The meta-Mean Ratio (mMR) of S100B blood levels in cases compared to controls was used as a measure of effect along with its 95% Confidence Intervals (CI). 20 studies were included totaling for 994 cases and 785 controls. Schizophrenia patients showed 76% higher S100B blood levels than controls with mMR = 1.76 95% CI: 1.44–2.15. No difference could be found between drug-free patients with mMR = 1.84 95%CI: 1.24–2.74 and patients on antipsychotic medication with mMR = 1.75 95% CI: 1.41–2.16). Similarly, ethnicity and stage of disease didn't affect results. Although S100B could be regarded as a possible biomarker of schizophrenia, limitations should be accounted when interpreting results, especially because of the high heterogeneity that remained >70%, even after carrying out subgroups analyses. These results point out that approaches based on traditional categorical diagnoses may be too restrictive and new approaches based on the characterization of new complex phenotypes should be considered.

Identifying biomarkers that can be used as diagnostics or predictors of treatment response (theranostics) in people with schizophrenia (Sz) will be an important step towards being able to provide personalized treatment. Findings from the studies in brain tissue have not yet been translated into biomarkers that are practical in clinical use because brain biopsies are not acceptable and neuroimaging techniques are expensive and the results are inconclusive. Thus, in recent years, there has been search for blood-based biomarkers for Sz as a valid alternative. Although there are some encouraging preliminary data to support the notion of peripheral biomarkers for Sz, it must be acknowledged that Sz is a complex and heterogeneous disorder which needs to be further dissected into subtype using biological based and clinical markers. The scope of this review is to critically examine published blood-based biomarker of Sz, focusing on possible uses for diagnosis, treatment response, or their relationship with schizophreniaassociated phenotype. We sorted the studies into six categories which include: (1) brain-derived neurotrophic factor; (2) inflammation and immune function; (3) neurochemistry; (4) oxidative stress response and metabolism; (5) epigenetics and microRNA; and (6) transcriptome and proteome studies. This review also summarized the molecules which have been conclusively reported as potential blood-based biomarkers for Sz in different blood cell types. Finally, we further discusses the pitfall of current blood-based studies and suggest that a prediction model-based, Sz specific, blood World Journal of Psychiatry W J P oriented study design as well as standardize blood collection conditions would be useful for Sz biomarker development.

Ovа studijа se bavi ispitivanjem potencijаlno zaštitinih efekata аgmаtina (AGM) nа razvoj oksidаtivnog/nitrozativnog stresa u selektivno osetljivim strukturama mozga pacova nakon davanja hlorpromаzina (HPZ). Sve ispitivane supstance aplikovane su u pojedinačnoj dozi intraperitonealno (i.p.). Životinje su podeljene na kontrolnu (K, 0.9 % fiziološki rastvor), HPZ (HPZ, 38.7 mg/kg TM), HPZ+AGM (AGM, 75 mg/kg TM odmah nakon HPZ, 38.7 mg/kg TM i.p.) i AGM (AGM, 75 mg/kg TM) grupu i žrtvovane dekapitacijom 24 h nakon odgovarajućeg tretmana. Rezultаti pokаzuju dа primena HPZ sa AGM značajno smanjuje koncentraciju leka u mozgu pacova u poređenju sа HPZ-životinjаmа (p<0.05). Aplikacija HPZ povećava lipidnu peroksidaciju (p<0.001 u korteksu, strijatumu i hipokampusu), koncentraciju nitrita i nitrata (p<0.001 u sva tri ispitivana regiona mozga) i stvaranje superoksidnog anjon radikala (p<0.05 u sve tri moždane strukture) u odnosu na odgovarajuću kontrolu, dok istovremeno utiče na oštećenje enzimskog antioksidativnog sistema (superoksid dizmutaza u korteksu i strijatumu p<0.05, odnosno hipokampusu p<0.001; glutation reduktaza u kori mozga i strijatumu p<0.001, odnosno hipokampusu p<0.05; katalaza u korteksu p<0.001, odnosno
