“…Echoing Pain et al (2017) in the call for holistic documentation, Sendall et al (2017) suggested that older people with chronic health co-morbidities could benefit from co-ordinated and integrated support services that "employ clinicians who provide holistic health reviews" (p. 65) and proactive planning and management (in contrast to single discipline or disease diagnosis or reactive treatment and siloed services). The important accuracy of information is problematic in aged care (Davis et al, 2016); as it is in disability services where there are multiple frameworks for information exchange and poorly developed information infrastructure (Dahm et al, 2019). Davis et al (2016), reporting on the development of a minimum dataset in aged care, considered the importance of efficient management systems and multidisciplinary involvement in providing supports, particularly focusing on quality, funding, communication, and continuity of care.…”
