Initial Evaluation of a Technique for Complete Cecal Bypass in the Horse

Abstract: Cecal bypass without ileal transection is technically simple and effectively bypasses the cecum. It also should intuitively require less time and have a reduced risk of contamination compared with complete cecal bypass with ileal transection.

“…This technique has been recently evaluated, and it could be considered an alternative surgical treatment for cecocolic intussusception that provides obstruction of the flow of ingesta aboral to the anastomosis without ileal transection. 10 Results of previous studies indicate that oversewing the cecal base for cecocolic intussusception is unnecessary to obtain good results. 3,6,9 Nevertheless, the intussusception of the cecum in the right ventral colon might not be completely sequestered from the rest of the abdominal cavity.…”

“…Greet, unpublished data). A novel technique using a TA-90 stapling device functionally to close the ileum without its complete transection was described recently (Symm et al 2006). This technique would be similarly applicable in cases of caecal dysfunction as the ingesta is prevented from flowing into the caecum.…”

“…Impaction is the most common surgical condition of the caecum in horses and has been reported to account for 40–55% of caecal problems, 5% of all intestinal impactions and 2% of referral colic cases 1,2–8 . Different published surgical approaches for the treatment of caecal impaction include typhlotomy, infusion of fluid with massage of caecal contents or enlargement of the caecocolic orifice, caecocolic anastomosis, incomplete or complete caecal bypass using jejuno‐ or ileocolic anastomosis, and the recently described complete caecal bypass without ileal transection 2,6–8 . Differences of opinion exist about which surgical technique is most preferable, likely related to the variability in the underlying pathogenesis of caecal impaction.…”

“…An experimental technique of a jejunocolostomy or ileocolostomy and complete occlusion of the more distal jejunum or ileum without transection was recently described in four normal horses 6 . The results suggest that occlusion without transection could be a viable alternative to traditional methods of caecal bypass.…”
