Injectables in the prostate

Abstract: Injection therapy remains a very promising minimally invasive surgical therapy for benign prostatic hyperplasia with increased attention from the urologic community in recent years. Further experience both with systematic laboratory and clinical trials investigation will be necessary before widespread clinical adoption.

“…Intraprostatic injection therapy for BPH is an inexpensive and simple procedure, with various results reported since the early 1900s [62][63][64][65]. Many injectable agents have been explored, with most reported to induce coagulative necrosis, followed by shrinkage of the prostate and resulting improvements in voiding dysfunction.…”

“…8 The key goals in the management of BPH are represented by controlling symptoms, preventing complications, and improving QoL. 9 While TURP still represents the gold standard treatment for BPH and despite many technical advances, the morbidity of this procedure has remained constant, in the range of 15% to 18%, with fluid absorption with dilutional hyponatremia and TUR syndrome, glycine toxicity, perforation, erectile dysfunction, retrograde ejaculation, and blood loss being the main areas of concern.…”

“…33 With the advent of novel injectables, this approach has experienced renewed interest. 34,35 The obvious advantages of a truly minimally invasive procedure include the absence of the adverse events associated with medical treatment, the lack of compliance issues that occur with long-term oral medication, the lack of an urgent need for anaesthesia and the minimal risk of technical adverse effects and complications. In times of dramatically rising health-care costs in an aging population, economic aspects are of critical importance and a cost-effective, easy-to-perform and efficient therapy performed in an office-based setting is desirable.…”
