Insulin management in chronic kidney disease: initiation, titration and monitoring of therapy

Abstract: Mr DM is a 60-year-old librarian with a history of type 2 diabetes and stage 4 chronic kidney disease. His GP reported that his diabetes control has been poor, despite being on the maximum dose of gliclazide. Michelle Lam and Adrian Coleman discuss the use of insulin in a patient with declining renal function.
