Intercostal Nerve Transfer to Neurotize the Musculocutaneous Nerve after Traumatic Brachial Plexus Avulsion: A Comparison of Two, Three, and Four Nerve Transfers

Abstract: The purpose of this study was to compare the outcomes of different numbers of intercostal nerve (ICN) transfers for elbow flexion and to determine whether age, body mass index (BMI), type of injury, and preoperative delay influence the outcomes. From 2004 to 2010, 32 (30 included) consecutive patients underwent ICN transfer to the musculocutaneous nerve following brachial plexus injury. Elbow flexion strength was evaluated according to the British Medical Research Council (BMRC) grading system. Of nine patient… Show more

“…Stabilization of the shoulder was achieved with the first two mentioned transfers at the initial procedure, while the conventional transfer of three intercostal nerves to the biceps brachii muscle was insufficient to restore elbow function (M1–2) in the present case (8). In the second procedure, it was necessary to do a free gracilis muscle transfer with the intention to improve elbow flexion (1416).…”

“…Intercostal nerves are commonly used to reinnervate muscles after a BPI with avulsion of spinal nerve roots (57); where two intercostal nerves should be enough for reinnervation of a muscle (8). Intercostal nerves can be harvested without any residual problems, e.g., pulmonary dysfunction (9).…”

“…At least two of them per recipient nerve are needed [24,124]. They have been used to re-innervate many nerves, like the AXN [89,92], MCN [24,105,106,[124][125][126], TLH [92,118,119], TDN [89,127] and SSN [90,91]. Their sensory branches can be used to recover some limb sensation, ameliorating the neuropathic pain.…”
