Background and Purpose: Fear and anxiety related to dental and healthcare procedures are common among young adults and can negatively impact dental and healthcare consumership, resulting in poor health outcomes. Purpose: This cross-sectional study aimed to examine young adults’ experiences of fear related to dental and healthcare procedures and the potential relationships between the two forms of fear experiences and demographic factors. Methods: Young adults (252) were recruited from a large public university and completed a survey about their dental and healthcare fear/anxiety as well as their confidence in healthcare professionals. Descriptive and correlational analyses were completed to describe the experiences of young adults and the relationships between variables. Results: Ten types of fear/anxiety were identified related to healthcare procedures, with the most common being needles and pain. Nine fears/anxieties were identified for dental visits, of which pain and sensory experiences other than pain were most common. Female participants were significantly more likely to report fear/anxiety related to dental procedures, and there was a statistically significant relationship between fear/anxiety related to healthcare procedures and that related to dental procedures. There was also a significant negative relationship found between confidence in healthcare providers and fear/anxiety related to healthcare procedures. Conclusions: Young adults experience fear/anxiety related to healthcare and dentist visits, and this fear/anxiety can impact their confidence in healthcare providers, such as primary physicians. The findings of the current study suggest primary care providers should be aware of potential fears/anxieties and offer patients appropriate supports.

ObjectiveOtolaryngologists play an increasing role in managing cough, but little data exists examining the demographics of this patient population and the referral patterns that influence their access to care. This study sought to elucidate these factors using a longitudinal, nationwide database to minimize sampling bias and identify trends representative of the national population.Study DesignNationally representative survey.SettingNational Ambulatory Medical Care Survey (NAMCS).MethodsVisits with a diagnosis and chief complaint of cough between 2005 and 2019 in NAMCS were examined. Univariable and multivariable analyses were performed to compare patient demographics between visits to surgical specialists, medical specialists, and primary care physicians.ResultsOtolaryngologists made up more than 84% of surgical specialist visits. There was a 0.52% [0.20%‐0.84%] increase per year in the proportion of visits attributed to surgical specialists. Based on a sensitivity analysis of the multivariable model, Hispanic patients (adjusted odds ratio, aOR: 0.88 [0.78‐0.99] vs White) and patients living outside of metropolitan areas (aOR: 0.77 [0.61‐0.99] vs living within) were less likely to see surgical specialists than primary care doctors for their cough. Patients who were referred (aOR: 1.47 [1.28‐1.72] vs not referred) and with chronic cough (aOR: 1.47 [1.23‐1.75] vs acute/subacute) were more likely to see a surgical specialist.ConclusionOtolaryngologists are increasingly called upon to evaluate and consider treatment for cough. Identifying patient groups with limited access underscores the need for enhanced education about otolaryngologists' roles and integrated care approaches to improve access to specialized cough treatment.Level of EvidenceLevel 4.

Purpose: The airway defense mechanism of cough is essential for human survival. Recognition is growing about the distinct role of speech-language pathologists in the nonpharmacological management of various cough disorders. This clinical focus article aims to deliver relevant education about normal and pathological cough to strengthen existing clinical knowledge and skills. Conclusions: The neuronal pathways underlying normal cough function are complex. Cough problems refer to either reduced or heightened cough stemming from various disease processes, and both clinical presentations have detrimental consequences. Current evidence supports that patients who suffer from cough dysfunction benefit from nonpharmacological interventions offered via speech therapy as an adjunct to medical treatments. Speech therapy strategies apply the higher cognitive components of cough by focusing on the deliberate modification and coordination of respiration. Future research priorities are positioned toward improving clinical outcomes for cough disorders.
