Interleukin 6 Strongly Correlates with Malnutrition Inflammation Score and it is Strong Predictor for Mortality in Hemodialysis Patients

Abstract: The annual morbidity and mortality rate in dialysis patients remains high despite the marked improvement in dialysis technology and patient care. Many hemodialysis (HD) patients 1-4 show increase in IL 6, which is indicated by increased circulating levels of nonspecific markers of inflammation. IL-6 plays an important role in inflammation, being the most potent inducer of the acute-phase response. 5 Nutritional status is important to analysis the CKD patients for morbidity and mortality 6,7 as well as in their… Show more

“…Similar results were also reported by Kaizu et where the average of IL-6 was 11.7 ± 2.8 pg/ml, 16.12 ± 34.1 pg/ml, and 45.2 ± 3.9 pg/ml [18], [22], [23]. Elevated of IL-6 can be caused by hypertension, adipocytes, insulin resistance, excess fluid, persistent infections, and dialysis procedures such as using bio incompatible membranes, non-sterile dialysis techniques, and back filtration [24], [25]. Several studies have shown that IL-6 do not increase during the hemodialysis process but rather than increase in 2 h after completion of hemodialysis [18], [25].…”

“…The immune system can be express in two grades; cellular and humeral immunity , cellular immunity composed of (T cell) and (Cytokines) , while the humeral immunity composed of ( B cell) which immunoglobulin [7] . (IL-6) is one of the main of acute phase response, which is noted high in patients with end-stage dialysis, and is also is powerful predictor of disease outcome [8] , as well as plays an important role in the relationship between inflammation, malnutrition, and cardiovascular disease (CVD) in patients undergoing hemodialysis (HD) [9] . (IL-18) is produced when the renal tubules damaged [10] , also may be used as a marker for the diagnosis of dystrophy function loss in renal dysfunction patients [11] .…”

“…This suggestion gave way to development of modified NUTRIC in which IL-6 is not included in the score [15], and most studies have excluded IL-6 [16][17][18]. However, there is abundant evidence suggesting that IL-6 is strongly correlated with malnutrition inflammation in hemodialysis patients [19,20], patients with lung cancer [21], patients with latent tuberculosis infection [22], and end stage renal disease patients [23]. Therefore, IL-6 can be considered as an important biomarker which cannot be easily ignored in patients hospitalized in ICU.…”
