Objective: To describe the clinical characteristics, complications, and risk factors for mortality of hospitalized patients with severe coronavirus disease 2019 (COVID-19) during intensive care therapy. Methods: This single-center retrospective cohort study included 149 patients with severe COVID-19 admitted to an intensive care unit (ICU) from April 2020 to August 2021. Demographic characteristics, medical history, underlying comorbidities, laboratory findings, complications occurring during ICU stay, and mortality were collected and analyzed. Survival and Cox regression analysis were performed to identify risk factors associated with mortality. Results: Out of 149 patients, 97 (65.1%) were women, and 67 of them were either pregnant or in the early postpartum period. The patients' ages ranged from 16 to 89 years, with a mean (SD) of 44.4 (18.9) years. The need for mechanical ventilation (72.5%) and the development of acute kidney injury (32.9%) were the most frequent complications. Thirty-eight (25.5%) patients died. The subgroup analysis of women showed that being pregnant/postpartum was not associated with a lower frequency of death. However, after adjustment, only increasing age (p<0.001) and a high AST-to-ALT ratio (p<0.001) were independent predictors of death. After adjustment, increasing age (p<0.001) and high AST-to-ALT ratio at admission (p<0.001) were independent predictors of death both in total sample and among women sub-group. High white blood cell count was also associated to death in the total sample analysis. Conclusion: The most common complications among severe COVID-19 patients in the ICU were the need for mechanical ventilation and acute kidney injury. Increasing age and a high AST-to-ALT ratio at admission were independent predictors of fatality among all patients and even among pregnant/postpartum women.

A pandemia COVID-19 teve consequências significativas para a população global. Entre os grupos que foram diretamente afetados, encontram-se as gestantes. É sabido que esse grupo apresenta maior vulnerabilidade aos efeitos da COVID-19. O objetivo da pesquisa analisar os casos de COVID-19 em gestantes hospitalizadas em um município no sudoeste mato-grossense. Estudo caráter descritivo, exploratório, retrospectivo e documental, com abordagem quantitativa. Os dados foram referentes ao período de abril de 2020 a abril de 2022, no município de Rondonópolis, Mato Grosso, coletados do painel de indicador COVID-19 do Estado de Mato Grosso. Incluíram todos casos de gestantes hospitalizadas notificadas por COVID-19 e variáveis clínicas e epidemiológicas. Foi realizada a análise descritiva dos dados dispostos em tabelas com frequências simples e absoluta e auxílio do software estatístico R. Foram realizados o teste de aderência de Qui-quadrado e o teste de correlação de Kendall, ao nível de significância de 5% (p-valor<0,05). Pesquisa aprovada pelo Comitê de Ética em Pesquisa. Durante o período de estudo, houveram total 50 notificações de COVID-19 em gestantes hospitalizadas. Predominaram casos nos meses de julho de 2020 e de 2021 (16%), faixa etária 30 a 39 anos (50%), cor parda (58%), não possuíam comorbidade (74%), hospitalizadas em leitos de enfermarias (72%) e desfechos cura/alta (82%) e óbitos (14%). Houve significância estatística das variáveis estudadas. Evidencia a importância de medidas de prevenção e da atenção da comunidade científica para este grupo com o intuito de prevenir agravos ao binômio mãe-filho. Objective: to identify research related to nursing care in Primary Health Care for obstetric patients and nursing mothers during the COVID-19 pandemic. Method: Integrative literature review in the LILACS, Scielo and MEDLINE databases using combinations "Pregnant Women OR Pregnancy AND COVID-19", "Breastfeeding OR Breastfeeding AND COVID-19" and "Primary Health Care AND Obstetrics". 28 articles were included in the search. Results: information analysed divided into 1) Vulnerabilities and impacts for women during pregnancy during the pandemic; 2) Potentials developed in the COVID-19 pandemic for pregnant and postpartum women; 3) Understand how to continue instructing women on breastfeeding in times of COVID-19 and 4) Protocols developed throughout the COVID-19 pandemic. Conclusions: the study presented looks at assistance to pregnant and breastfeeding women during the pandemic and guides the work for this audience today.
