Internal hernia of the transverse colon

Abstract: A unique abdominal internal hernia is described. A 30-year-old man suffered intestinal obstruction and strangulation due to incarceration of the transverse colon in the subphrenic space. This phenomenon is contrasted with Chilaiditi's syndrome (hepatodiaphragmatic interposition).

“…5 Most colonic internal hernias in literature involve the transverse colon, likely due to its redundancy and mobility through a mesenteric pedicle, and occur due to congenital defects. 6,7 This patient had not had his right colon mobilized, and it therefore remained in its normal embryological plane. He had significant adhesions from the ileal conduit to the right colon and lateral abdominal wall.…”

“…Colonic herniation through the foramen Winslow and right subphrenic space are very rare types of internal hernia. 9 Direct roentgenograms, CT, 10 and color flow echocardiography have been helpful for the diagnosis. Endoscopic evaluations and contrast roentgenograms are preferable approaches in elective patients.…”

“…İnsidansı yaşla artar, erkek/kadın oranı 4/1'dir. Çoğunlukla asemptomatik kalırlar, hafif karın ağrısından akut intermitan barsak obstrüksiyonuna kadar bir dizi semptom oluşturabilirler (3).Bu makalede chilaiditi sendromu tanısı koyduğumuz 2 hastayı sunduk. Bunlardan biri ayakta direk batın grafisinde şüpheli serbest hava görülmesi nedeniyle akut batın ön tanısıyla acil servisimize sevk edilmişti, diğeri ise darp şikayetiyle gelen direk grafisinde insidental olarak yakaladığımız bir vakaydı.…”
