Interobserver Agreement for Confocal Imaging of Ampullary Lesions

Abstract: The overall interpersonal agreement on pCLE for ampullary lesions was poor. The interobserver agreement was not substantially improved for experienced raters. Further standardization of pCLE image criteria is needed for ampullary lesions. Standardized training may improve interrater reliability to an acceptable level.

“…Current application for CLE in gastrointestinal (GI) endoscopy includes Barrett's Esophagus, biliary strictures, diagnosis and follow-up of colonic and gastric lesions [92]. Other possible future applications of CLE might also include the diagnosis of breast diseases [93], celiac disease [94], and ampullary lesions [95]. One of the first fields of application of CLE was represented by Barrett's esophagus; the research in this field has greatly contributed to the standardization of confocal image interpretation.…”

“…En lesiones ampulares se ha intentado mejorar la evaluación diagnóstica utilizando pCLE pero con resultados pobres por falta de criterios específicos, y con mala concordancia interobservador, por lo que para esta patología faltan más estudios (41,42).…”

“…CLE generates images that elucidate cellular architecture and microvasculature at a level comparable to that achieved with traditional histology [11,12]. In particular, several studies have shown that this technology is useful for determining pathology in a variety of gastrointestinal tissue sites, including Barrett's esophagus [13][14][15], and lesions in the duodenum [16], colonic mucosa [17,18], and pancreatic biliary tract [19][20][21]. Moreover, it has been reported that CLE, incorporated into gastrointestinal endoscopy, is a useful approach for diagnosing various malignancies, including gastric [22,23], esophageal [24,25], colorectal [26,27], and pancreatic cancers [28].…”
