Interobserver agreement in the assessment of muscle strength and functional abilities in Guillain‐Barré syndrome

Abstract: In studies of Guillain-Barré syndrome, functional deficit is usually assessed according to a functional scale consisting of several categories. The level of interobserver agreement in this scoring method is not known; furthermore, this method seems to be insensitive when applied to bedridden and artificially ventilated patients. We have developed an additional score (MRC-sumscore), reflecting muscle strength in general. Both scoring methods, tested in Guillain-Barré patients, have an almost perfect interobserv… Show more

“…Muscle strength can be quantified using the Medical Research Council (MRC) sum score, a validated ordinal scale which grades muscle weakness in six bilateral muscle groups from upper and lower limbs between 0 (no contraction) and 5 (normal strength) (TABLE 3). Adding these subscores results in a maximum sum score of 60 (369). ICUAW was defined as MRC sum score for muscle strength to be Ͻ48, provided no other cause of weakness could be identified (147).…”

“…Information on MMN was collected using the protocol of Cats and colleagues as a template [Cats et al 2010 [Kleyweg et al 1991]. The following muscles were assessed on both sides: upper arm abductors, elbow flexors, wrist extensors, hip flexors, knee extensors and foot dorsal flexors.…”

“…6 Medical Research Council (MRC) sum score was used for valuing the muscle strength from 0 to 5 in proxi- mal and distal muscles in upper and lower limbs bilaterally; score ranged from 40 (normal) to 0 (quadriplegic) and by Hughes et al disability score for GBS. 7,8 We further subdivided Hughes grade 5 to 5A and 5B based on requirement of ventilator. (Table 1).…”
