A majority of the unsafe abortions are performed by untrained birth attendants or quacks leading to complications in a large proportion of these cases. Complications like bowel injury, bladder injury, uterine perforation, and septic abortion are mostly caused by unskilled hands and are detected immediately or within few days of the procedure, owing to the need for tertiary level care. Here we present a very interesting case of unsafe abortion induced by a Ryle's tube in a 32-year-old lady, which was diagnosed five years after the procedure. Considering its atypical presentation, it is the first case of its kind in the literature. The details of the case and its management are described along with appropriate pictures.

BackgroundAn unsafe abortion is defined as a procedure for terminating an unintended pregnancy carried out either by a person lacking the necessary skills or in an environment that does not conform to minimal medical standards or both. Majority of these unsafe abortions are carried out in rural areas of developing countries, usually by unskilled persons who do not have proper knowledge of the anatomy of reproductive organs and in unhygienic environments thus leading to various complications.Case presentationWe discuss the case of a 21 year old female who presented in septic shock after she underwent an unsafe abortion of an 11 weeks pregnancy with uterine wall perforation and bowel injury that required resection.ConclusionUnsafe abortion is an important public health problem which accounts for a significant cause of maternal mortality and morbidity in resource poor countries. A high index of suspicion of clandestine abortion with ensuing complications should prevail when faced with a woman of child bearing age with the triad of vaginal bleeding, amenorrhea and pelvic sepsis.

Objectives: To find frequency of common complications among patients presenting with induced septic abortion attending OPD of Hayatabad Medical Complex Peshawar. Study Design: Descriptive cross-sectional study. Setting: Department of Obstetrics & Gynecology Hayatabad Medical complex Peshawar. Period: 6 October, 2016 to 6 April, 2017. Material & Methods: Through a cross sectional validation study design, 123 pregnant female patients with induced septic abortion having a gestation period of 20 to 22 weeks were included in the research study. After their consent, detailed clinical examination and history of patients were taken. SPSS version 10.0 was used for analysis of the collected data. Descriptive statistical data like mean + Standard deviation was measured for age, gravidity & parity. Likewise, frequency & percentage was calculated for hemorrhage, diffuse peritonitis and severe anemia. Results: As per Common Complications, frequencies and percentages for hemorrhage was recorded in 31 (25.20%) patients, diffuse peritonitis was recorded in 49 (39.83%) patients while severe anemia was recorded in 26 (21.13%) patients. However, 17 patients (13.82%) had no complications other than septic abortion. Conclusion: Our study concluded that the mishap of septic-induced abortion is totally preventable. The definitive commitment to women’s health can be achieved through effective contraception and by strengthening the family welfare services. Positive results can be achieved by discouraging repeated terminations of pregnancy.
