Intracranial Subdural Abscess Caused by Actinomyces meyeri Related to Dental Treatment: A Case Report

Abstract: Intracranial subdural abscess is a rare condition. Although brain abscess is often reported in relation to dental infection, reports of intracranial subdural abscess are few. Actinomyces spp. forms part of the normal flora of the oral, gastrointestinal, and genital tract, and is rarely the cause of intracranial infection; moreover, the pathogen Actinomyces meyeri is very rare. “…Still, an empiric regimen against staphylococcus , streptococcus , anaerobes, and gram-negative bacilli must be provided (metronidazole and ceftriaxone or cefotaxime which can enter the cerebrospinal fluid). Furthermore, anti-epileptic drugs should be given in view of the high incidence of perioperative seizures and steroids to decrease inflammation and cerebral edema [ 1 , 3 , 5 ]. As seen in our case, if SE is due to tooth infection, experts believe dental extraction may be required in addition to antibiotics [ 10 ].…”

“…Brain abscess can be challenging to diagnose on clinical symptoms alone as its presentation could be due to other etiologies. Common presentations include headaches, fever, altered mental status, decreased consciousness, seizures, cerebellar involvement, as well as the constellation of abdominal pain, nausea, vomiting, and diarrhea [6, 16,23]. The characteristic triad of headache, fever, and focalization only occurs in 20-30% of cases [14].…”
