Intraductal papillary mucinous tumors and mucinous cystic tumors of the pancreas: imaging

Abstract: Most cystic lesions of the pancreas are nonneoplastic and inflammatory in nature. However, approximately 5%-15% of cystic pancreatic masses may be neoplastic. Among the cystic neoplasms are the mucin-producing tumors, both the intraductal papillary mucinous neoplasms and the mucinous cystic neoplasms. Their imaging features on contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI) can assist in the differentiation of these lesions. The imaging findings of both intraductal papillary muc… Show more

“…In terms of the unique pathological characteristics of IPMT, the appropriate surgical resection for IPMT has been debated for years 3. Although appropriate management may depend on the location and size of the IPMT, and, most importantly, whether the IPMT is benign or malignant, the inconclusive discussions may be explained, in part, by the fact that preoperative diagnosis of the tumor is occasionally dificult 8. Even though IPMT can be diagnosed preoperatively by imaging analyses, diagnosis as to whether the tumor is malignant or benign is generally impossible.…”

“…MRI shows MPD dilation, and is better to detect intramural nodules and their connection to MPD, than Endoscopic Retrograde Cholangiopancreatography (ERCP). Despite the excellent images obtained by CT and MRI they do not exhibit high diagnostic accuracy in determining the presence of malignancy [60].…”

“…Tiny cysts may occur in the wall of MCA and are sometimes diagnosed as mural nodules [28]. These characteristics can be clearly demonstrated by MRCP [60]. Communication between a PCL and the MPD is less common, but this finding does not make a definite diagnosis of MCN [25,28,60].…”
