Intraepithelial CD8+tumor-infiltrating lymphocytes and a high CD8+/regulatory T cell ratio are associated with favorable prognosis in ovarian cancer

Abstract: The favorable prognostic effect of intraepithelial CD8 ؉ TILs did not correlate with concurrent expression of NY-ESO-1 or MAGE antigens. We conclude that intraepithelial CD8 ؉ TILs and a high CD8 ؉ ͞Treg ratio are associated with favorable prognosis in epithelial ovarian cancer.cancer testis antigen ͉ CD8 ϩ T cell

“…These markers were not associated with improved outcomes in our relatively small cohort of highgrade serous carcinomas but other series with greater case numbers have consistently demonstrated immune cell infiltrates to be a favorable prognostic factor. 18,19,22,24,59 TP53 abnormalities (loss of expression, which correlates with nonsense mutations or deletions, or overexpression, which correlates with missense mutations) were essentially ubiquitous in high-grade serous cancers with BRCA1 or BRCA2 abnormalities and also were associated with favorable outcome. Thus BRCA mutations, immune cell infiltrates, and TP53 abnormalities co-vary in high-grade serous carcinomas.…”

“…16,17 Tumors from women with high-grade serous cancers frequently show a host immune response, and the presence of cytotoxic T lymphocytes, and T-regulatory cells is consistently associated with favorable prognosis. [18][19][20][21][22][23][24] Germline mutations in BRCA1 and BRCA2 are present in B18% of ovarian cancer patients with high-grade serous carcinoma. [25][26][27] When combined with BRCA deficiencies that result from somatic mutations or epigenetic silencing, it appears that up to half of all high-grade serous ovarian cancers (hereditary and sporadic) have BRCA dysfunction.…”

“…The prognostic significance of the host immune response, as defined by the presence of tumor-infiltrating lymphocytes, has been studied in ovarian cancer [8][9][10][11] and other cancers [12][13][14][15][16][17][18] Zhang et al 10 identified intratumoral (later named intraepithelial) CD3 þ T-cell infiltration to be an independent prognostic factor in 186 advanced ovarian cancers. Curiel et al 8 demonstrated that infiltration of CD4 þ CD25 þ regulatory T-cells carried a worse prognosis in ovarian cancer.…”

“…6-9 An important part of the battle between tumor cells and the immune system is fought within, but not confined to, the tumor microenvironment. CD8+ T cell infiltration into EOC has a positive effect on chemotherapy response, 7 , 8 while infiltration with suppressive immune cells such as regulatory T cells (Tregs), myeloid derived suppressor cells (MDSC) and M2 macrophages is associated with a lower response to chemotherapy in EOC. 10-13 In addition, a strong influx of MDSC in ascites is associated with poor survival.…”
