Qualitative Identification of Intervention Preferences to Support Men’s Engagement and Retention in TB Care in South Africa

Globally and in South African specifically, men account for 56% and 62% of all tuberculosis (TB) cases, respectively. Men are at increased risk of not accessing TB testing or treatment, and having poor treatment outcomes. Unfortunately, no interventions exist to address these issues. Toward the development of targeted, patient-centered TB care and support interventions, we used semistructured interviews to explored men’s social network composition, TB testing behaviors, disclosure and treatment support, clinical experiences, and TB’s influence on daily living. Data were analyzed using a thematic approach guided by the Network Individual Resource Model to identify mental and tangible resources influential and preferred during engagement in TB treatment. Men emphasized the desire for peer-to-peer support to navigate TB-related stigma and unhealthy masculinity norms. Men advocated for awareness events to educate communities about their challenges with TB. Men strongly suggested that interventions be delivered in familiar locations where men congregate. Since 2022, no TB treatment support interventions have included the preferred components or delivery modes described by men in our study. To improve men’s TB-related health outcomes, the global TB community must identify and address men’s unique challenges when designing interventions.

Background: HIV testing is the first step for linkage to HIV prevention or treatment services. Facility-based HIV testing is the most utilised method, but faces challenges such as limited work space and human resources. Digitally supported HIV self-testing (HIVST) provided in clinics shifts testing to the client, potentially empowering the client, and addresses such constraints.Objectives: The study primary objective was to determine the feasibility of integrating digitally supported HIVST into the clinic. Secondary objectives were to describe HIV testing volume, populations reached, and antiretroviral treatment (ART) initiation.Method: We conducted an analysis of prospectively collected data during implementation of digitally supported HIVST in two healthcare facilities based in South Africa from June 2019 to September 2019. We described implementation and client characteristics using HIVST and compared testing before and during implementation.Results: During the 4-month implementation period there were 35 248 client visits. A total of 6997 (19.9%) of these visits involved HIV testing. Of those testing, 2278 (32.5%) used HIVST. Of the 2267 analysed, 264 (11.6%) were positive: 182 (12%) women and 82 (11%) men. Of those, 230 (95.4%) were confirmed HIV positive and 150 (65%) initiated ART within 14 days. During a four-month pre-implementation period, 14.5% of the clients tested for HIV. Compared to the pre-implementation period, we observed a 25% increase in HIV testing.Conclusion: Digitally supported HIVST increased the number of clients completing HIV testing in the health facility, without a need to significantly increase staff or space. Facility-based digitally assisted HIVST has the potential to increase HIV testing in high HIV prevalence clinic populations.

This research compares the intervention effectiveness of face-to-face counseling and telenursing method on the Anti-Retroviral medicine consumption adherence for individuals suffering from HIV/AIDS. The method: researchers explored the data from EBSCOHost, science direct, Proquest, springer nature, and PubMed within the period of 2018 and 2022. The exploration found 1.521 journal articles plus their abstracts. Out of 1.521 journal articles, the researchers found 10 articles examining the effectiveness of counseling-based digital media for both computers and applications on mobile. Results:10 studies used the mHealth platform, virtual counseling, digital media, computer-based technology, and mobile. The assessments on the ARV medicine consumption adherence applied CD4 and viral load checkup. On the other hand, the researchers found the assessment of the ARV medicine consumption adherence applied Electronic Dose Monitoring, EDM, to examine the missing doses. The researchers found electronic pill monitoring or eCAP to examine ARV medicine consumption adherence. The other assessment was the Visual Analog Scale, VAS, to report the side effects. Then, the last assessment was an autonomous self-report to remind the medical visits via phone calls. Conclusion: face-to-face counseling allows counselors to examine the clients' responses, transmission risk, and ARV medication hindrances directly. The counselors could also find the solution to deal with the problems and observe the allergic reactions to the medication or other complaints due to immunosuppressive. The researchers also concluded that technology could facilitate the service distance, save time and cost, provide accessible health, provide nursing health care, provide immediate responses, and become the support system for the clients to consume the medicines.
