Is laparoscopic approach for wandering spleen in children an option?

Abstract: Wandering spleen is a rare entity and in the paediatric age group 10% cannot be salvaged for which splenectomy is the only option. Of the 90% that can be pexied, the literature has favoured the application of meshes followed by the extraperitoneal pockets and omental pouch. Laparoscopic splenopexy is feasible, with no reported conversions or complications.

“…However, if there is no blood flow to the spleen even after detorsing, a splenectomy is performed. 4 At the time of our patient's presentation, the spleen was completely infarcted with evidence of ischemia on CT scan. The likelihood of return flow to the spleen was deemed unlikely.…”

“…The other presentations are usually related to the mass effect of the enlarged spleen and patients could present with GOO, bowel obstruction, pancreatitis and urinary symptoms [3,6]. • In some cases it is reported to be associated with some other disorders like gastric volvulus [7] and distal pancreatic volvulus [8]. • Ultrasound is one of the imaging modalities to investigate patients whom we suspect had wandering spleen.…”

“…• Partial infraction of a wandering spleen might necessitate partial splenectomy and splenopexy or splenectomy and splenic implantation [6,11]. • The spleen might get fixed by different methods [8,9]. Fig.…”

“…A mi eseteinkben három különböző technikát választottunk a lép rögzítésére. Korábban egyértelműen a nyitott műtéti technika volt jellemző, a mai trendek azonban egyre inkább a minimálinvazív technikák felé mutatnak [14]. Az első laparoszkópos splenopexiát Hirose és mtsai publikálták 1998-ban, azóta egyre szélesebb körben alkalmazzák világszerte [15].…”
