Is Supine Rest Necessary before Blood Sampling for Plasma Metanephrines?

Abstract: Background: The impact of blood sampling in sitting vs supine positions on measurements of plasma metanephrines for diagnosis of pheochromocytoma is unknown. Methods: We compared plasma concentrations of free metanephrines in samples from patients with primary hypertension obtained after supine rest with those obtained in the sitting position without preceding rest. We also assessed the effects on diagnostic test performance retrospectively in patients with and without pheochromocytoma, and we calculated cost-… Show more

“…The conclusion in the report by Sawka et al [9] that measurements of plasma free metanephrines provide good diagnostic sensitivity, but suffer from suboptimal specificity, did not take into account the effects of seated posture [15]. In that study tests of urinary metanephrines were also restricted to spectrophotometric measurements of total metanephrines (i.e, the sum of metanephrine and normetanephrine), which showed relatively good specificity, but poor sensitivity.…”

“…Next, we examined the same parametares in individual metanephrine as follows. The sensitivity and specificity were 0.638 and 0. results [8]. This position can help to prevent the secretion of catecholamines due to anxiety or stress, which may not originate from the tumor.…”

“…A blood sample for measurement of metanephrines should ideally be taken after supine rest in a quiet room for at least 20-30 min, because samples obtained in sitting position without preceding rest provide a lower diagnostic accuracy (44). This is particularly important for the analysis of normetanephrine, the metabolite most sensitive to sympathoadrenal activation.…”
