Is That Another Thornwaldt's Cyst on M.R.I.?

Abstract: 20% of 200 M.R.I. head scans displayed increased signal in the midline posterior-superior nasopharynx in the position of the pharyngeal bursa. Though these likely represent a number of different entities, Thornwaldt's cysts may be more common than previously realised.

“…It is not certain that there is a direct or indirect link between the fossa navicularis and the cyst of Tornwaldt, because no study to date backs this association [31]. Tornwaldt's cyst remains generally asymptomatic, but the larger the cyst is, the higher its risk to become symptomatic [27]. The symptoms include posterior nasopharyngeal discharge, occipital cephalalgia, halitosis, stiffness of cervical muscles and dysfunction of the Eustachian tube with a feeling of fullness in the ear.…”

“…MRI signal of mucous retention cysts is usually high on T2-and low on T1-weighted images (Fig. 8) [27], but if the cyst content becomes rich in proteins, a high T1 signal may be observed (Fig. 9).…”

“…MRI is now considered the best radiographic imaging study to diagnose the tornwaldt's cyst as the findings are highly characteristic [8]. On MRI, it may have high signal on both T1-and T2-weighted sequences, presumably because the cyst contains a high concentration of protein, or blood products from prior hemorrhage, or both [10][11][12]. Post contrast studies usually demonstrate peripheral enhancement of the nasopharyngeal mucosa [8].…”

“…MRI of the nasopharynx is a sensitive method for detecting and evaluating cystic lesions of the nasopharynx (8,9). The lesion usually has a characteristic high signal intensity on T2-weighted and intermediate to high signal intensity on T1-weighted MRI imaging (10,11). The variation in signal intensity on T1-weighted images may be related to differences in protein content or hemorrhage in the cyst (12).They are variable in size, ranging from a few millimetres to a few centimetres in diameter, but are typically 2 -10mm in size (12,4).…”
