Is There a Benefit to Highly Crosslinked Polyethylene in Posterior-stabilized Total Knee Arthroplasty? A Randomized Trial

Abstract: Background Polyethylene wear and osteolysis remain a concern with the use of modular, fixed-bearing total knee arthroplasty (TKA). A variety of highly crosslinked polyethylenes (XLPs) have been introduced to address this problem, but there are few data on the results and complications of this polyethylene in posterior-stabilized knee prostheses. Questions/purposes In a randomized prospective study design comparing standard polyethylene (SP) tibial inserts with XLP inserts, we asked the following questions: (1)… Show more

“…Experimental studies have indicated improved wear performance compared with conventional UHMWPE, [8][9][10] and clinical studies have indicated a comparable or improved performance. 4,11 However, a trade-off between wear resistance associated with the level of cross-linking, and the mechanical properties of the material has been highlighted. 12,13 Asano et al 14 demonstrated an almost linear relationship between wear resistance and radiation dose, highlighting a corresponding reduction in mechanical properties.…”

“…55 Totally, six studies (three randomized clinical trials and three non-randomized clinical trials) were included with a follow-up period of 2-6 years (Table 3). [56][57][58][59][60]64 Outcome measures included total number of operations, reoperations for prosthesis loosening, radiolucent lines, osteolysis, prosthesis loosening, mechanical failures related to tibial PE, postoperative total knee score, and functional scores. No significant differences were seen in any of the measured variables.…”
