Is there a post-PPH syndrome?

Abstract: Some long-term studies critically examining PPH have come to fruition. A majority of respondents continued to perform PPH. Nearly half of these agreed that there is a "post-PPH syndrome" relating to postoperative morbidities. The most disturbing morbidity was lasting perineal pain of unexplained etiology demanding challenging management. Persistent bleeding from hemorrhoidal disease distal to the staple line requires further management and raises the question as to the use of PPH as a permanent remedial proced… Show more

“…5,23 The presence of persistent hemorrhoidal disease, sphincter spasm, rectal spasm, high anal resting pressures, anal fissures, retained staples, and fibrosis around the staple line, wound dehiscence, and sepsis, were also identified as contributing factors to excessive and/or prolonged pain. 31,48 It was also suggested to occur more frequently in males and people with grade 4 hemorrhoidal disease, or those with high anal sphincter pressures. 31,48 A low threshold for suspicion of complications should exist in patients suffering prolonged and severe pain following a stapled hemorrhoidopexy.…”

“…In the present study, although the overall complication rate was 23%, majority of the complications were mild and were resolved during the follow up. Persistent pain after SH is considered chronic when lasting one week after surgery and it seems that the majority of patient recovers from it [12,27,30]. Prevalence of persistent pain after SH ranges from 1.6% to 31% [31].…”
