Purpose: To perform a systematic review with best evidence synthesis examining the literature on the relationship between occupational loading tasks and knee osteoarthritis (OA). Methods: Two databases were searched to identify articles published between 1946 and April, 2011. Eligible studies were those that (1) included adults reporting on their employment history; (2) measured individuals' exposure to work-related activities with heavy loading in the knee joint; and (3) identified presence of knee OA (determined by X-ray), cartilage defects associated with knee OA (identified by magnetic resonance imaging), or joint replacement surgery. Results: A total of 32 articles from 31 studies met the inclusion criteria. We found moderate evidence that combined heavy lifting and kneeling is a risk factor for knee OA, with odds ratios (OR) varying from 1.8 to 7.9, and limited evidence for heavy lifting (OR ¼ 1.4-7.3), kneeling (OR ¼ 1.5-6.9), stair climbing (OR ¼ 1.6-5.1), and occupational groups (OR ¼ 1.4-4.7) as risk factors. When examined by sex, moderate level evidence of knee OA was found in men; however, the evidence in women was limited. Conclusions: Further high-quality prospective studies are warranted to provide further evidence on the role of occupational loading tasks in knee OA, particularly in women.

Objectiv e: The aim of this study is to investigate the effect of workload on musculoskeletal degeneration. Background:It is important that workers maintain a health body for a long time as a measure of aging workers. In order do so, it is necessary to find out if workload can effect on musculoskeletal degeneration.Method: Epidemiological studies on workload or occupation and disc degeneration, osteoarthritis of knee were identified through database and bibliography searches.Results: Epidemiological studies showed that workload certainly effect on musculoskeletal degeneration. But we couldn't know which of individual factors and occupational factors further contribute to the musculoskeletal degeneration. And we could find that studies on workload and genetic factors were very few. In addition, there was also very few studies that it was possible interaction between individual factors as obesity and occupational factors as heavy manual lifting.Conclusion: Our reviews suggest that it need to conduct study between workload and genetic factors for musculoskeletal degeneration. This further study can identify attributable risk of workload for musculoskeletal degeneration, and contribute to the measure of aging workers.Application: This paper can help to establish research plan for the measure of aging workers.

Purpose. This study investigated the EMG characteristics of muscles crossing the knee and the kinetics of the lower extremity during side-slope walking and other activities of daily living. We studied the difference in EMG data of the medial gastrocnemius and vastus lateralis muscles bilaterally and the relative rotation of the thigh to leg. Methods. Eleven outdoor workers (47.3 ± 13.9 years old) were recruited for this study. Participants walked on a 0° flat surface, 5° and 10° side-sloped surfaces, 10° inclined treadmill and ascended stairs. The EMG activity and rotation about a vertical axis during stance phase were analyzed. Results. Except for minor variations, ANOVA showed no significant difference in EMG activity between the walking surfaces, furthermore, the relative rotation of thigh-to-leg showed little or no differences between the variables. Multivariate ANOVA showed p-values between 0.1602 and 0.9943 when comparing the EMG data of all side-sloped surfaces. The relative rotation of the thigh to the leg showed p-values of 0.7837 and 0.9813 when comparing the left 0° to 10° and right 0° to 10°, respectively. Conclusions. The results of this study indirectly indicate that when considering rotation about a vertical axis and EMG activity, there is little difference in knee joint loading.
