Isolated aortic valve replacement in North America comprising 108,687 patients in 10 years: Changes in risks, valve types, and outcomes in the Society of Thoracic Surgeons National Database

Abstract: Morbidity and mortality of isolated aortic valve replacement have fallen, despite gradual increases in patient age and overall risk profile. There has been a shift toward bioprostheses. Women, patients older than 70 years, and patients with ejection fraction less than 30% have worse outcomes for mortality, stroke, and postoperative stay.

“…In spite of that, randomized controlled trails as well as meta-analysis studies (Aris et al, 1999;J. M. Brown et al, 2009;Dogan et al, 2003;Murtuza et al, 2008) were not able to show a convincing superiority of MIAVR over CAVR.…”

“…Over the past two decades, minimally access cardiac surgery has been increasingly adopted with the goal to reduce the invasiveness of surgical procedures and aiming in the same time to give the same quality, safety and results of the conventional surgery. In spite of that, randomized controlled trails as well as meta-analysis studies (Aris et al, 1999;J. M. Brown et al, 2009;Dogan et al, 2003;Murtuza et al, 2008), were not able to show a convincing superiority of MIAVR over CAVR Aim: This study aimed to determine whether the MIAVR via upper partial sternotomy offers significant advantages in the postoperative outcomes (morbidities and mortalities) over CAVR via standard full sternotomy.…”

“…Generally speaking, the average age of patients undergoing aortic valve replacement is 69 years [2] and the currently available biological valve substitutes (porcine or bovine) have a longterm function of 15-18 years. The majority of patients therefore, can be offered excellent valve substitutes; however younger patients (approximately 70 000 worldwide each year are younger than 60) need alternative options.…”
