Isolated Cleft Lip and Palate: Maxillary Gap Sign and Palatino-Maxillary Diameter at 11–13 Weeks

Abstract: Objective: To evaluate the maxillary gap sign and describe markers for the first-trimester diagnosis of isolated cleft lip and palate (CLP) at 11–13 weeks. Methods: Firstly, this was a prospective assessment of 1,087 fetuses including 5 cases of isolated CLP in 2 centers which were referred for the 11–13 weeks scan. Secondly, intra- and interobserver variability of the maxillary gap sign was evaluated for observers R.L. and A.B. in 2 sessions (affected cases vs. 50 normal fetuses in each session) to reduce the… Show more

“…The diagnosis of a facial cleft can be made in the first trimester by evaluation of the retronasal angle, the frontonasal space distance, and the continuity of the maxilla. [6][7][8][9] Associated abnormalities of the central nervous system, such as holoprosencephaly, should be identifiable between 11 and 13 weeks of gestation. 10…”

“…We will not exaggerate the value of our findings and do not want to state that one measurement/method is superior to the other evaluated markers. We strongly believe that, using the combined approach we suggest, the vast majority of so-far-notdetected but potentially detectable abnormalities will be detected with low false-positive rates as we have shown in several studies in comparison to previous approaches that resulted in a detection rate of 0-13% (Lachmann et al 2017). We would like to hear your opinion incorporating this information.…”

“…The mid sagittal view of the face shows the typical reduced ossified area of cleft palate [22], presenting with both recently published signs: the enlargement of maxillary gap and the shortening of palatino-maxillary diameter [10], [11]. Anterior to the cleft palate the processus alveolares maxillae present thickened and buckled [18].…”

“…Directly under the ethmoid bone, the aperture of the eyelid is situated in the midline, caudally the face is flat, the nose is absent. Using a different insonation angel maxillary gap measuring 2.7 mm (normal <1.5 mm) and palatinomaxillary diameter ( Figure 3) measuring 1.2 mm (<5 th centile) as a marker of cleft palate are displayed [10], [11].…”
